Responsibilities Assesses all patients assigned and establishes and implements the Plan of Care Adheres to policies and procedures addressing the scope of services outlined in both Infusion nursing services, pharmacy services and state laws and regulations Adheres to INS and IgNS standards of practice, CDC guidelines, scope and standards of practice, and code of ethics as defined by American Nurses Association Collaborates with pharmacy staff, sales partners, ordering provider and patient to develop and monitor goals of patients’ therapies Completes patient care documentation in place of service including patients’ home or AIS Manages vascular access devices, to include insertion of peripheral infusion devices and ability to draw labs via peripheral venipuncture Educates patients and caregivers Evaluates progress toward goals and makes recommendations to physicians for changes in the Plan of Care based on patients’ needs Coordinates nursing care with home health and other community agencies as indicated Participate in on-call rotation as scheduled per branch protocol Complete expense reports monthly tracking mileage and other miscellaneous incidental expenses Complete annual competencies and mandatory training within assigned due dates Supervisory Responsibility: No Qualifications Graduate of an accredited school of nursing; Bachelor of Science in Nursing preferred Valid nursing license in state(s) of practice Minimum of one (1) year of experience as a Registered Nurse Previous infusion and home health care experience preferred Extensive experience in IV therapy preferred Current CPR certification Flexible schedule with the availability to work a variety of shift models Ability to work effectively within a multidisciplinary team Effective verbal and written communication skills Valid driver’s license and auto insurance Must meet company standards for driving record as well as driver age requirement Percentage of Travel: 100%-Regular driving to patient’s homes for visits is required as part of this position **To perform this role will require constantly sitting, standing, walking, bending, reaching, crouching, stooping and typing on a keyboard with fingers. Nurse Manager, Emergency Department

Full Time - Nights - 10 hour shifts Primary Function: The Nurse Manager in a collaborative relationship with the Vice President of Patient Care Services is responsible for the planning of clinical services to assure efficient and appropriate delivery of care to the unit’s specific patient population. The Nurse Manager will adhere to organizational philosophies and assure that quality care is delivered in a cost-effective manner. The Nurse Manager will be responsible for the 24-hour delivery of nursing care in the department. Takes appropriate corrective action to resolve and/or assure complaint is not reoccurring. Facilitates and evaluates inter-shift communication and cooperation. Plans and meets periodically with the Vice President for the purpose of setting unit goals and objectives. Develops a Performance Improvement Program to monitor the quality and effectiveness of care delivered in the department. Assures the Joint Commission standards are met. Actively supports and communicates the goals of Patient Care Services in a concise clear manner. Holds staff accountable to meet goals of Patient Care Services. Serves as Role models professional development. Responsible for own professional development, setting goals and striving to achieve them through available resources. Maintains management competencies through attending seminars, in-services and/or reading journals. Develops agendas and schedules monthly staff meetings for all shifts. Job Qualification Master’s degree required or Bachelor’s degree along with enrollment in a Master’s program with job related experience may be considered. Certification in administration or related specialty is preferred and required within two years of hire. Now Hiring - Licensed Practical Nurse The Licensed Practical Nurse will be responsible for, but are not limited to: Observes and documents the physical, mental and emotional needs of the patient and family on an ongoing basis Consults with the Hospice Nurse regarding the patient's status and symptom management at appropriate levels Delivers physical and psychological care as prescribed by the attending physician, as supervised by the Hospice Nurse and as approved by the IDC team Monitors and documents patient/family input and response to care Maintains continuity of patient care through ongoing communication with families and other health care professionals Participates in Interdisciplinary Team Meetings and all other meetings as assigned Maintains sufficient and appropriate home care supplies for adequate patient care management Collection of documentation and paperwork as requested by the Hospice Nurse.
