This manager will manage RN case managers, CHWs, and BHAs, in accordance with established Compass (formerly Care Model) and ICP Programs, as well as all UHC requirements. Manage productivity and expected outcomes across the teams, engage staff. Must be a critical thinker and have experience in complex care management, chronic conditions, and behavioral health management to manage, lead and advise the teams as a subject matter expect. This role also requires submission of state reports, presentations and public speaking. Data trending, extracting, capturing and analysis are also an essential element of this role. Incorporates the concepts of the Hospital's Customer Service Program into all aspects of patient care activities. Accountable to the Nurse Manager or designee for all activities. Education High School Diploma or Equivalent Required Trade School Completion of an approved Nursing Assistant Program or current Nursing student with satisfactory completion of at least one clinical rotation Required or Combination of relevant education and experience may be considered in lieu of degree Required Experience 1 year experience in a Nursing Assistant role Preferred Licenses Basic Life Support Required 
Assists in the coordination and facilitation of patient care and hospital operations on all shifts. Provides supervision, leadership, direction, and clinical support to assure the delivery of safe, quality patient care. Functions as administrative resource for hospital operations in the absence of Administration.
