JOB DESCRIPTION Job Summary Provides medical assistant support for Care Connections team. Responsible for contacting members following appointments/services rendered to ensure member understanding and navigation of the health care system, and coordination support for follow-up care. Also provides assistance for other administrative clinical support tasks. Contributes to overarching strategy to provide quality and cost-effective member care. Essential Job Duties • Coordinates care between members and providers after appointment/service delivery to support member navigation of the health care system, needs related to follow-up care and highest quality care/desired member health outcomes. • Communicates with Care Connections providers to clarify orders/requests and assists with escalation of issues to appropriate team(s) when unable to resolve independently. • Supports processing and follow-up for care management assignment and primary care provider referrals for high-risk members. • Escalates member issues related to prior authorizations for medications and durable medical equipment. • Communicates lab results and other exam findings with members and primary care providers. • Communicates with nurse practitioners on member health issues to ensure timely follow-up and response. • Supports care coordination and intervention by sending electronic messages/faxes to primary care providers. • Closes gaps in health care by preparing member lab result letters and coordinates follow-up on lab results. • Fulfills nurse practitioner supply orders and maintains adequate nursing supply inventory. • Performs administrative tasks such as telephone and email response, documentation follow-up and updates to member records. • Facilitates follow-up calls to members to ensure positive member experience/outcomes and ensure needs are met. • Assists with special projects under the direction of clinical leadership. Required Qualifications • At least 1 year of health care experience preferably in a family practice, home health, primary care, palliative care or hospice setting, or equivalent combination of relevant education and experience. • Graduate of an accredited medical assistant (MA) program. • Demonstrated understanding of patient care, medical terminology, coding procedures, reference tools and appropriate clinical pharmacology for medical assistant practice scope. • Excellent customer service skills. • Ability to work independently or as part of a team. • Ability and desire to meet or exceed Care Connections clinical integration and departmental metrics and conjunctively provide excellent customer service. • Strong attention to detail, ability to multi-task, prioritize, meet deadlines and demonstrate follow-through. • Utilizes critical-thinking skills to identify issues, problem-solve and demonstrate initiative. • Empathy for senior, disabled, income challenged and vulnerable populations. • Ability to build positive working relationships with peers and effectively manage conflict. • Strong verbal and written communication skills. • Microsoft Office suite/applicable software program(s) proficiency, and ability to learn new programs. • Ability to lift up to 50 lbs. Preferred Qualifications • Certified Medical Assistant (CMA). • Experience working with an electronic medical record system (EMR), preferably Epic. • Experience with underserved populations facing socioeconomic barriers to health care. • Bilingual in Spanish, Korean, Mandarin, Cantonese or Vietnamese. Description Patient Business Services Provide clinical review, audit, analytical, and quality improvement support for utilization management, medical necessity determinations, and denial prevention and response efforts across the health system. As the Utilization Management Nurse Auditor, you will conduct concurrent and retrospective case reviews, support denial appeals, analyze utilization and denial trends, and collaborate with Care Coordination, Clinical Documentation Integrity, Revenue Cycle, and Physician Advisor teams. This role helps strengthen level-of-care determinations, clinical documentation, payer compliance, and operational and financial performance. In this role, you will: • Review concurrent and retrospective clinical denials to assess admission status, level of care, length of stay, medical necessity, and other factors contributing to denials. • Prepare clinical summaries and supporting documentation for first- and second-level appeals, support payer, RAC, and Medi-Cal audit responses, and collaborate with Revenue Cycle and Physician Advisor teams to strengthen appeal strategies. • Analyze denial and audit findings to identify trends, root causes, documentation gaps, avoidable delays, and opportunities to improve utilization management performance. • Evaluate cases using established utilization review criteria, including InterQual, MCG, organizational guidelines, and payer requirements, and escalate complex or questionable cases as appropriate. • Partner with Physician Advisors, Care Coordination teams, and clinical staff to improve documentation supporting medical necessity, status designation, and accurate level-of-care determinations. • Prepare reports, dashboards, presentations, case summaries, trend analyses, and recommendations for leadership and Utilization Management Committee review. • Support performance improvement initiatives focused on length of stay, avoidable days, denial rates, status accuracy, utilization outcomes, and data integrity. • Serve as a clinical resource while collaborating with clinical and operational leaders across the health system to support utilization management, clinical quality, patient safety, care progression, discharge planning, and continuous improvement efforts. Responsibilities The Clinical Supervisor supports administrative responsibility for the department service functions and works collaboratively with the unit Director and/or Manager. In the absence of the unit Director and/or Manager, the Clinical Supervisor may assume the administrative scope of the unit assigned. Develops and implements departmental plans, including performance improvement activities and compliance with current federal and/or state regulations. The Clinical Supervisor prepares, conducts and/or oversees department(s) orientation and training of personnel. Communicates with staff, physicians, and administration both written and verbally. The Clinical Supervisor is responsibility and accountable for the quality of staff and functions during their shift. Assists the Department Director in the daily operations of the unit, directing, supervising, and evaluating personnel, ensuring the proper use of time, equipment and staff. Ensure the efficient functions of the department. Performs direct patient care, only as needed, within the scope of practice. Continually assess care and direct staff to make immediate changes in the plan of care. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. Current and valid California Registered Nurse license2. Bachelor Degree in Nursing (BSN) preferred.3. Current Basic Life Support certificate (BLS) upon hire and maintain current.4. Minimum of three year of Charge Nurse or equivalent experience preferred.5. Based on areas the following may be required: a.
