Department Description The Adult Internal Medicine Unit cares for medicine patients ranging from young adults to the elderly presenting with a wide variety of diagnoses. Patients found on this unit are often admitted for acute exacerbations of complex chronic conditions with a focus on pulmonary, gastrointestinal, sickle cell, renal failure, medical-psychological, and endocrine conditions. In addition, patients are admitted for diagnostic work-up and treatment of new-onset infectious disease processes. Continuous video camera monitoring and cardiac telemetry are also available, if needed, to provide comprehensive, safe patient care. Collaboration between our physician colleagues (teaching services and attending services) and multidisciplinary teams contributes to the high functioning teamwork found on this unit. The unit is also supported by the Geriatric Clinical Nurse Specialist (CNS), ACE RN Program Coordinator, Medical/Surgical CNS/Educators and an Oncology CNS. This cohesive team approach allows our nurses to provide a high standard of innovative care to patients. Nursing based research that enhances nursing or improves patient care and safety is actively supported here. Job Description The Center for Pain Medicine is a high-volume specialty clinic with multiple satellite clinics that focuses on a variety of chronic pain diagnoses. The Center for Pain Medicine provides holistic care to include procedural intervention, infusions, psychology services and medication management. The Center for Pain Medicine also collaborates with the department of Psychiatry to provide infusion therapy for a select group of patients. The clinical nurse provides care for patients with multiple or complex pain management needs. They provide advanced clinical skills in assessing, treating, and evaluating patients with chronic pain. They facilitate an interdisciplinary and collaborative approach to the treatment of patients and will assist with patient inquiries in person, via phone, and electronic health record messaging. PEDIATRIC HOSPICE RN CASE MANAGER JOB DESCRIPTION Overview The Hospice Pediatric RN Case Manager will work with our Pediatric team. Primarily responsible for providing professional nursing care to assigned patients. With input from the IDT (interdisciplinary team), patient and family, assesses, develops, implements and evaluates the plan of care. Ensures appropriate communication and plan of care coordination to all internal/external customers All successful candidates must be fully vaccinated, subject to certain limited exceptions required by applicable law. DEGREE OF TRAVEL: • Travel required for patient visits. • Occasional travel for educational purposes. QUALIFICATIONS: • Must have Associates Degree, Bachelor Degree preferred. • Must have one year of acute or hospice pediatric experience • At least 12 months experience with 1-2 years preferred in acute care. • Current CA RN license. • Current CA driver’s license with proof of insurance. • Current CPR/BLS • Prefer certification as hospice nurse. • Pediatric experience preferred. • Proof of recent TB test result • Bilingual preferred.
