The inpatient program admits patients five years of age or older who meet the criteria for inpatient admission. The program provides 24-hour-a-day, multidisciplinary, intensive treatment for patients with acute psychiatric disorders, cognitive impairment, and patients with autism and intellectual disabilities with varying medical complexities. The inpatient behavioral health services offer people with acute psychiatric crises who urgently require a safe environment, a supportive and structured milieu, and intensive psychiatric and physiological therapeutic intervention. The treatment program combines these elements into a unique and effective treatment program. We provide management of acutely ill patients with lift-threatening conditions requiring complex assessment, high- intensity therapies and interventions, and continuous nursing vigilance. Trinitas is proud to be the only hospital in New Jersey with a separate unit to serve this population. The psychiatric inpatient 27 bed closed unit is an involuntary unit. Patients are committed to being inpatient for treatment and can have varying medical complexities. The psychiatric inpatient 21bed open unit is a voluntary unit. Patients are able to sign themselves in for treatment and can have varying medical complexities. Our staff are cross trained to work all the units within our inpatient behavioral health department. GENERAL JOB SUMMARY An exempt clinical position where the nurse practitioner (NP) is responsible for providing direct patient care. The NP participates as a leader of the skilled nursing facility (SNF) care team. Visits managed care and fee-for-service patients at skilled and long-term levels of care in designated SNFs facilities. Provides appropriate evidence-based geriatric medicine. Coordinates care with hospitalists, primary care physicians and care managers. Makes home visits as directed by the medical staff to meet patient needs and provide continuity of care. ESSENTIAL JOB FUNCTIONS Maintains privileges in multiple Nursing Homes as directed by ACA Maintains license and malpractice insurance Consults supervising attending as needed Documents patient visits electronically at least 90% of the time Participates in documentation and other quality improvement programs Available via phone weekdays 8am- 7pm and when on call. The training should include a written examination and competency demonstration. A Hemodialysis LPN may: • Administer bolus of medications by IV push in a chronic outpatient hemodialysis treatment center, excluding first dose administration which is administered by an RN. The RN will access and care for patient’s initial catheter use. Subsequently, and following the MSHS CVAD care policy, the LPN can: • Identify and set up supplies necessary for accessing a CVAD. • Initiate treatment with a CVAD. • Draw blood specimens from a CVAD. • Perform CVAD dressing change and exit site care. • Monitor and adjust flow rates of central venous catheter access device/line. • Intervene if there is a catheter/blood line disconnection or other complications associated with a CVAD. • Provide CVAD care at the end of dialysis treatment.
