Pay Range: $19.32 - $24.13 Scheduled Weekly Hours: 40 Position Overview Prepares patients for examination by performing preliminary physical tests; taking blood pressure, weight, and temperature; reporting patient history summary. Cleans exam rooms, obtains lab specimens. Assists the physicians and other staff as needed. Selects appropriate equipment prior to collection to streamline process. Verifies specimen orders per department policy Prepares exam rooms for patient encounters and assists LIP as requested. Maintains appropriate levels of supplies and all relevant forms for any testing performed and maintains statistics as assigned Performs thoroughly all clerical duties as needed, including but not limited to patient registration, answering phones, scheduling appointments, initiating, maintaining and filing medical records and timely distribution of reports. Performs electrocardiograms and other relevant tests including point of care testing following established policies and procedures and universal precautions. Verifies patient information by interviewing patient; recording medical history; confirming purpose of visit. Documents medical information in electronic medical records including medication and refill information and patient call documentation, keeps rooms stocked and organized. Organizes paperwork and filing. Coordinates with front office personnel and ancillary staff in professional and positive manner in adherence of policies and procedures in support of patient service. Assists in front office functions as assigned including but not limited to: Checkout/in duties with claim creation and assuring complete process of discharge paperwork, referrals, patient education, appointments, insurance eligibility etc. Performs other selected clinical and clerical duties that assist in the delivery of primary health care and patient care management within any CH office location under the direction of a provider and manager in accordance with the objectives and policies of Capital Health (CH) and respective state laws governing such action and activities. Cross-trained to perform Patient Reception Rep job duties as needed. New Jersey Prescription Monitoring Program (NJPMP)- uploads a notarized "request for access" form and a copy of their State-accepted Certified Medical Assistant certificate; performs NJPMP monitoring under the direction of the LIP (Licensed Independent Practitioner). Performs injections, subcutaneous or intramuscular; performs duties in accordance with Capital Health policies. Performs other duties as assigned. The Assistant Team Lead RN will assist with the day-to-day coordination of clinical services, support RN Case Managers, help ensure quality patient care, and promote compliance with hospice regulations, company policies, and clinical standards. This position combines direct patient care, clinical support, staff mentoring, and leadership responsibilities and is ideal for an experienced hospice RN who is ready to take the next step into a leadership role. This position will oversee Burlington County. Responsibilities Assist the Team Lead with daily clinical operations and coordination of the hospice nursing team. Provide guidance, support, and mentorship to RN Case Managers and other clinical staff. Assist with assigning and coordinating patient visits based on patient needs, staffing, and geographic coverage. Monitor patient care to promote continuity, quality, and compliance with the hospice plan of care. Assist nurses with clinical questions, challenging patient situations, symptom management, and changes in patient condition. Participate in patient care and make nursing visits as needed to support the clinical team. Assist with reviewing clinical documentation for completeness, accuracy, and compliance. Help monitor timely completion of assessments, recertifications, care plans, orders, and other required clinical documentation. Assist with identifying changes in patient condition and ensuring appropriate communication with physicians, hospice providers, patients, and families. Support coordination between nursing, aides, social work, chaplain, volunteers, providers, and other members of the interdisciplinary team. Serve as a clinical resource and role model for nursing staff. Participate in interdisciplinary group meetings and other clinical meetings as assigned. Respond appropriately to urgent clinical situations and assist with escalation of patient-care concerns. Qualifications Current, unrestricted Registered Nurse (RN) license in the State of New Jersey. Associate Degree in Nursing required; BSN preferred . Minimum of 2–3 years of professional nursing experience . Previous hospice, home health, palliative care, or end-of-life care experience strongly preferred. Previous leadership, charge nurse, preceptor, or supervisory experience preferred. Current CPR/BLS certification. Valid driver's license, reliable transportation, and appropriate automobile insurance. Strong clinical assessment and critical-thinking skills. Excellent written and verbal communication skills. Strong organizational and time-management abilities.
