This role provides comprehensive nursing care across crisis respite, intensive case management, and community transition services. Provide health education to individuals, families, and direct support professionals. Coordinate hospital discharges and community reintegration services. Arrange follow-up care with primary care providers, specialists, and community resources. Serve as a clinical liaison between providers, families, and program staff. Participate in interdisciplinary treatment planning and case conferences. Train and mentor Direct Support Professionals (DSPs) on health-related procedures and emergency response. Maintain accurate and timely clinical documentation. Qualifications Current unrestricted New York State Registered Nurse (RN) license. Minimum of two years of nursing experience. Experience working with individuals with IDD, behavioral health, crisis care, or community health is preferred. Knowledge of OPWDD, OMH, and DOH regulations is preferred. Strong assessment, crisis intervention, care coordination, communication, and documentation skills. Ability to work independently and collaboratively within a multidisciplinary team. Position Summary Serenity Hospice Care is seeking a compassionate, experienced, and organized Assistant Team Lead RN to support the clinical leadership team and provide guidance to our hospice nursing staff. The Assistant Team Lead RN will assist with the day-to-day coordination of clinical services, support RN Case Managers, help ensure quality patient care, and promote compliance with hospice regulations, company policies, and clinical standards. This position combines direct patient care, clinical support, staff mentoring, and leadership responsibilities and is ideal for an experienced hospice RN who is ready to take the next step into a leadership role. This position will oversee Bergen/Morris County. Responsibilities Assist the Team Lead with daily clinical operations and coordination of the hospice nursing team. Provide guidance, support, and mentorship to RN Case Managers and other clinical staff. Assist with assigning and coordinating patient visits based on patient needs, staffing, and geographic coverage. Job Description The Registered Nurse in the Sexual Health Center provides direct patient care, coordinates treatment plans, collaborates with interdisciplinary teams to ensure quality outcomes, maintains clinical documentation, adheres to safety protocols, and supports patient education and advocacy. The RN will work partly in the ambulatory fixed site at 200 South Street in Morristown, partly in the Morristown Medical Center Emergency Department, and partly in the community in an Atlantic Mobile Health vehicle and at community-based organizations through Northern New Jersey including, but not limited to, Morris, Sussex, and Passaic Counties. The Sexual Health Department in a small, interdisciplinary team that primarily promotes public health initiatives funded through grants and awards. Public health initiatives include prevention, testing, and treatment for HIV, HCV, and sexually transmitted infections; harm reduction health programming such as preventative care, acute and chronic disease management, and wound care; education and counseling to promote safer sex and use strategies. Principal Accountabilities Patient Assessment: Perform systematic, comprehensive, and accurate assessments. Assess potential discharge needs during the initial assessment process. Administers medications to patients and monitor effects of medications and treatments and report adverse effects in a timely manner. Assess patient for pain level. School LPN Nurse Needed in the Bronx 2 days per week, 9am-5pm! Scope of Services Consultant will coordinate and oversee the health care services across the 2 sites. This includes: -Completing all screenings including vision, height, dental, weight and lead. Dental and vision will be monitored and charted if completed prior by the physician. Weight will be monitored and recorded if completed prior and nurse will do follow up after 3 months. We are seeking a Registered Nurse (RN) for their Crisis Respite/Transitional Program (CRTP), Intensive Case Management (ICM), and Critical Time Intervention (CTI). The Registered Nurse (RN) provides comprehensive nursing services across the CRTP, ICM, and CTI. This full-time role supports individuals with dual diagnoses of intellectual and developmental disabilities (IDD) and severe mental illness (SMI) needs by integrating skilled nursing care, crisis stabilization, transitional planning, and intensive care coordination. The RN collaborates with interdisciplinary teams to promote health, safety, dignity, and successful community integration. The RN ensures that complex medical, behavioral, and psychiatric needs are met during the transition from an inpatient setting to the community. Salary : $95,000 per year plus benefits. Schedule: Monday - Friday 9am-5pm Essential Responsibilities (including but are not limited to): Clinical Care -Screen individuals who are being considered for admission to the CRTP and the Transitional Respite Program. Active member of the "intake” team. Conduct initial and ongoing nursing assessments for individuals admitted to CRTP. -Oversight of self-medication administration program, monitor therapeutic response, and document outcomes. -Provide direct nursing interventions such as wound care, diabetes management, respiratory illness support, and chronic disease oversight. -Monitor early signs of medical or psychiatric crisis and initiate appropriate interventions. -Regularly reviews medication administration records, logs documenting vital signs, elimination, glucose readings, weight, menses, and other records as individually specified. -Reports significant issues to appropriate clinicians and team members, including the need for ordering medications, renewing or updating prescription orders, and people-supported specific drug information sheets. -Monitor and manage medication storage and disposal procedures in accordance with federal and state law. -Develop and monitor health goals that are person specific. Review, update, and revise these documents to ensure they are reflective of the current person supported status. -In compliance with OSHA mandates, monitors infection control, environmental changes, and ensures that all guidelines regarding universal precautions are appropriately followed. -Order and maintain necessary first aid and medical supplies as needed. -24/7 availability via phone for consultation and basic direct care. Case Management & Transitional Support -Partner with ICM teams to address both medical and behavioral health needs in service planning. -Deliver health education and coaching to individuals, families, and direct support staff to promote independence and wellness. -Facilitate hospital discharge and community reintegration through CTI, ensuring continuity of medical, psychiatric, and social support. -Coordinate follow-up care with primary care providers, specialists, and community-based resources. Collaboration & Training -Serve as the clinical liaison among providers, families, and program staff. -Participate in interdisciplinary treatment planning and case conferences. -Train and mentor direct support professionals (DSPs) on health-related tasks and emergency response. -Maintain timely and accurate documentation in compliance with OPWDD, OMH, and DOH regulations.
