Essential Job Responsibilities: The RN provides care to patients who have been diagnosed with a terminal illness. The Hospice RN regularly reviews and updates the plan of care. Performs prescribed medical treatments, including pain management and symptom control, conducts assessments and evaluations, provides education and supportive care to patient and family. Completes an initial, comprehensive, and ongoing assessments of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). Provides professional nursing care by utilizing all elements of nursing process. Assesses and evaluates patient’s status by: Writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs, participating in revising the plan of care as necessary. Uses health assessment data to determine nursing diagnosis. Develops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient as appropriate per assessment and plan. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. Acts as Case Manager when assigned by Clinical Supervisor/Nursing Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Completes, maintains, and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. Communicates with community health related persons to coordinate the care plan. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. Provides and maintains a safe environment for the patient. Assists the patient and family/caregiver and other team members in providing continuity of care. Works in cooperation with the family/caregiver and hospice Interdisciplinary Group Members to meet the emotional needs of the patient and family/caregiver. Attends interdisciplinary group meetings. Position Qualifications Registered Nurse with valid CA state license. Hospice experience preferred. At least one year of recent hospice experience along with a strong foundation of acute care experience. Maintains a current CPR certification. Valid driver’s license, auto insurance, reliable transportation, and willingness to drive to patient locations. Qualifications • Master's or doctoral degree in nursing • Current NP License within the State Responsibilities • The Nurse Practitioner (NP) is an experienced licensed professional who works in collaboration with the Hospice Medical Director/Physician to deliver hospice and/ or palliative care to patients and families • Performs prescribed medical treatments, including patient examination, diagnoses, medication prescription, pain management and symptom control; conducts assessments and evaluations, and provides education and supportive care to hospice teams, patients and caregivers • Performs comprehensive patient examinations, evaluations, assessments and diagnosis • Assesses for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action to control and manage patient symptoms and needs • Makes adjustments to the already established plan of care and appropriate services, provides appropriate follow- up to all patients • Demonstrates flexibility in scheduling, and readily accepts new and emergency assignments as requested • Performs timely face to face assessments with patients • Demonstrates positive interpersonal relation in communicating with physicians and co-workers to promote quality patient care • Completes all documentation in an accurate, legible and timely manner • Provides quality patient care consistent with standards of nursing practices • Complies with hospice policies and procedures, laws and regulations and standards of practice This position will cover Sonoma, Marin, and Napa counties.

Certified Registered Nurse Anesthetist - Santa Rosa Hospital - Full Time Varies

Job Summary: Selects and administers intravenous, spinal and other anesthetics prior to and during surgical or medical procedures, as a member of the anesthesia care team and in accordance with Kaiser Permanente policies and procedures. Monitors patient reactions and vital signs during and after anesthesia and takes remedial action when warranted. Submits postoperative report on patient condition, and maintains records of anesthetics administered. Performs procedures outside of the OR, in accordance with KP policies and procedures, established competency, and MD order. Essential Responsibilities: Conducts and/or reviews anesthetic evaluation of patients prior to surgical proceedings, deliveries or other pertinent medical procedures in hospital setting; consults with anesthesiologists and/or surgeons. Reviews patients charts, including permits, pre-operative medications, lab work, and any other significant variations, such as hearing aids, dentures, contact lenses, or other prostheses, in accordance with KPs policies and procedures and the departmental policy. Evaluates patients to identify apparent difficulties with airway control, etc. Ensures that all pre-anesthesia evaluations are documented thoroughly according to policies and standards. Explains tests, procedures and disease processes to patients and their family as indicated; explains the major anesthetic risks to patients as requested and documents that the risks are explained to patients. Develops the care regimen of assigned patients; reviews the care regimen with the supervising physician and gains concurrence; implements the anesthesia plan; starts IV solution, administers selected anesthetic, choice of anesthetic consistent with patient safety, surgical need, possible post operative complications, and patients history and conditions. and evaluates and manages the patient, according to departmental policy. Participates with post anesthesia care unit personnel in a patient safety oriented interface when patients are transferred from the surgery suite. Monitors patients vital signs, such as blood pressure and observes any significant physical changes; informs anesthesiologist and/or surgeon of patients condition and takes any necessary remedial action such as infant resuscitation when necessary. Mixes solutions to required concentrations, such as pentothal, succinylcholine, vaso-active drugs etc. Keeps current accurate and complete record of procedures performed and patients conditions immediately prior to the administration of anesthesia according to established standards. Performs procedures done outside of OR, such as intubation, arterial line placement, etc., in accordance with policies and procedures, competencies, and physician order. Charts details of procedures done on progress notes. Kaiser Permanente conducts compensation reviews of positions on a routine basis.
