Position Summary: DAY SHIFT Weekend Warrior $8.00 premium pay The Registered Nurse is responsible for performing a variety of duties to provide quality nursing care to residents and to coordinate total nursing care for residents; for participating with the Director of Nursing and the Unit Managers in establishing specific goals; for ensuring compliance with all operating policies and procedures and Michigan Department of Health & Human Services regulations and Federal Health Care Administration regulations; for communicating with and supervising appropriate personnel; for providing daily reports and other periodic reports; and for performing all the duties of a Certified Nurse Aide when required. Principal Duties and Responsibilities: Assess resident conditions to implement intervention and develop Care Plans. Accurately record resident observations in PCC and follow through on Care Plans. Assure facility medical records policies and procedures are adhered to. Make regular rounds of residents independently and with a physician; observe and evaluate resident symptoms, progress, and reactions to treatments and medications and implements interventions as necessary. Provide preventative, supportive, maintenance, and rehabilitative care directed to the physiologic and psychosocial needs and well-being of residents. Provide care to residents by performing a variety of treatments, including changing dressing, cleansing and/or irrigating wounds and incisions, performing Foley Catheter irrigations, IV therapy, and administering tube feedings, chest tubes and peritoneal dialysis. Monitor and assist CNAs with personal resident care duties such as bathing, feeding and assisting with ambulation, and other routine accommodative services for resident comfort and well-being. Receive reports from outgoing nurse and make rounds to see each resident daily and as needed. Administer medications to residents according to the Public Health Code, Nursing Department Policies, and standards and procedures and as prescribed by the physician; notify appropriate clinical and/or nursing personnel of medication contraindications. Advise residents and/or responsible persons regarding changes in medications, treatment plans, and resident condition. Manage and document emergency care of residents, coordinating communications to physicians, responsible persons, DON and Administrator, hospital personnel, emergency transport, and other appropriate personnel. Perform comprehensive and focused nursing assessments in conjunction with changes in condition and scheduled programs. Admit, transfer, and discharge residents. Requests and assures supplies and equipment are available to perform care and treatment as required by the patient prior to or at the time of admission. Assure completion of weekly, periodic and discharge summaries. Perform quality improvement activities as assigned by the Director of Nursing. Monitor rate of accident/incidents occurrences and infection rate on the unit and develop suggestions for corrective action. Weekly skin assessment to monitor occurrence of pressure injuries or other skin conditions, providing the Director of Nursing written data on the number of occurrences. Report resident and staff complaint and all unusual occurrences to the Director of Nursing or Administrator and any allegation of Abuse/misappropriation or neglect to the administrator immediately. Participate in periodic regulatory surveys by the State of Michigan and participates in the preparation of the Plan of Correction response to an inspection survey. Complete and report resident injury to Unit Manager. Fully understands all aspects of residents’ rights, including the right to be free of restraints and free of abuse. Is responsible for promptly reporting to the charge nurse or administrator incidents or evidence of resident abuse or violation of residents’ rights. Know and support facility philosophy, standards, policies and procedures. Communicate with department heads on an as-needed basis to problem solve and plan for more effective patient care. The RN is responsible for providing individualized care that encourages the resident to maintain or attain the highest practical physical, mental, and psychological well being. Observe and evaluate each resident on your assigned unit. Administer and document current medications and treatments. Identify resident problems and emergency situations, initiate immediate “life-saving” measures in the absence of a physician. Deliver direct nursing care to residents and provide clinical supervision of the care staff assigned on your unit. Perform and document a comprehensive assessment on each assigned resident and develop care plan and more. General Summary: Under the direction of the Office Manager, the Medical Assistant performs administrative and clinical duties to care for patients in an ambulatory setting. Administrative duties include maintaining medical records, handling patient calls and completing forms. Clinical duties include measuring and recording vital signs and medical histories, preparing patients for examination and administering medications as directed by physician or advance practice providers. The Medical Assistant plays an integral role in the care team model. Knowledge, Skills and Abilities: High school degree (or GED) required. Current Certification as Medical Assistant required. Basic Life Support (BLS) required by 3/1/2025. Previous experience as Medical Assistant preferred Excellent interpersonal skills and ability to effectively work with the medical team Ability to effectively communicate with patients and staff while demonstrating a high degree of diplomacy and tact Ability to operate medical equipment safely and correctly Knowledge of infection prevention & control standards and practices Flexible, organized and able to function under stressful situations Ability to understand, interpret and apply instructions, guidelines and procedures Culturally sensitive and able to work with ethnically diverse patient populations Ability to prioritize and work on multiple tasks Intermediate experience with computers and information systems Essential Functions and Responsibilities: Ensures all relevant reports, tests results, labs and correspondence are available prior to the patient’s appointment. Escorts patients to exam room, interviews patients, measures vital signs and document all information in patient’s medical record. Reviews patient’s medical record to determine needed services and whether referrals are in process or have been completed since last visit. Assists physician or advance practice providers during examination or procedure. Provides patient with information and instructions as authorized by physician or advanced practice provider. Keeps exam room stocked with adequate medical supplies, maintains instruments and prepares them for sterilization and storage. Provides clerical support which may include completing forms/requisitions, answering phones, sending/receiving medical records, obtaining lab/x-ray reports, hospital notes, and referral information. Cleans rooms between patient visits in accordance with infection prevention and control standards. Administers medications and immunizations under authorization and supervision of physicians and/or advance practice providers. Processes messages from patients and front office staff to physicians and advance practice providers. Processes messages and call backs to patients, medical offices and pharmacies. Processes medication refills according to protocols. Maintains all logs and required checks (i.e. refrigerator temperature, emergency medications, expired medications, oxygen, sterilization fluid charge etc.) Performs EKG’s and simple (CLIA waived testing) medical office laboratory and screening tests. Collects and processes blood samples and other non-blood specimens for the diagnosis, treatment and prevention of disease. Participates in process improvement activities to enhance patient care and service.
