Description The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care 1. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. 2. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. 3. Integrates relevant assessment and intervention skills in the delivery of nursing care. 4. Communicates effectively and professionally with patients, family and all members of the Interdisciplinary Patient Care Team. 8. Manages assignments within the Care Delivery Model of Modified Primary Nursing and demonstrates allocation of material resources effectively. Patient Experience 1. Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions including, but not limited to, Admission Welcome, Bedside shift Report, HELP (High Risk Medications, Equipment, Lines, Drains, Airway, Pain and Plan of Care), AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. 2. Considers the patients values, preferences, expressed needs and knowledge in all aspects of care. 3. Establishes and maintains a therapeutic relationship with the patient and family. 4. Uses evidence-based practices to increase understanding of patient's perceptions of care. 5. Initiates service recovery for patients and family members who have concerns and escalates to Clinical Nurse Manager/Assistant Nurse Manager/Nursing Administrator or Patient Relations as needed. 6. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. 7. Supports staff use of evidence-based practices to increase understanding of patient's perceptions of care. 8. Conducts Purposeful Hourly Rounding (PHR) on assigned patients addressing the 4 Ps. Quality and Safety 1. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. 2. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. 3. Implements process improvement strategies based on institutional, evidence-based ensure and procedures. 4. Engages in formal and informal peer and institutional review processes. 5. Collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities and nursing research studies. 6. Contributes to quality and safety practices and required compliance measures. 7. Implements principles of high reliability to identify and avoid high risk behaviors to provide a safe environment for patients. 8. Escalates potential safety hazards or gaps from best practice following institutional chain of command protocol. 9. Utilizes institutional information technology by documenting nursing practice to support quality and performance improvement initiatives. Operations 1. Performs charge nurse duties as assigned. 2. Coordinates with the Clinical Coordinator/Clinical Nurse Manager/Assistant Nurse Manager/Nursing Administrator to ensure supplies are appropriately utilized and inventory is sufficient for patient care. 3. Escalates material or staffing resource deficits to charge nurse, Clinical Nurse Manager/Assistant Nurse Manager or Nursing Administrator. 4. Demonstrates patient focused and cost-effective approaches to patient care in terms of equipment, staff, supplies and all other resources. 5. Documents in an accurate and thorough manner in compliance with hospital, regulatory and legal requirements and standards of care. 6. Facilitates patient throughput on a shift basis. Illustrative Duties Graduate Nurse (T&RC) Prepares or assists with preparation of nursing care plans consistent with individuals’ comprehensive treatment plans and medical needs. Records notes and maintains individuals’ medical records and nursing care plans, including nursing care, medications, and treatments, reactions to care and treatment, intake and output of liquids and solids, and vital signs. Triages individuals by inquiring about illnesses, injuries, and symptoms, and records findings and observations. Delegates responsibility for appropriate nursing activities, skills, and/or procedures to nursing and direct care staff; and makes clinical rounds to assess individuals’ nursing care needs and observe and evaluate the provision of nursing services. May perform the full range of supervisory duties over Graduate Nurses (T&RC), Teaching & Research Center Licensed Practical Nurses, and direct care staff; provide information to Registered Nurse Supervisors 1 (T&RC); and prepare or contribute to the preparation of reports and performance evaluations for lower-level staff involved in nursing care activities. Registered Nurse 2 (T&RC) Positions perform duties in each of the three functional areas described below. Direct Individual Care: Provides nursing care to individuals in an acute care environment requiring complex nursing skills; uses advanced knowledge and skill in nursing assessment, planning, implementation, and evaluation activities. Observes and responds to individual emergency situations, and sets priorities for care based on individual acuity, preferences, and the possibility of effective intervention. Implements nursing interventions in response to changes in individual status and unusual circumstances. Interprets assessment data to determine if individuals are at physiological or psychosocial risk. Takes steps to decrease stress and increase the effectiveness of individual coping mechanisms; uses effective communication strategies with individuals, families, staff, and the general public. Assesses individual status upon entry to the hospital and throughout hospitalization. Identifies implications of individuals’ statuses and prioritizes data collection based on individuals’ immediate needs. Develops and maintains care standards knowledge bases for specialty care areas and develops care guidelines for unit-specific individual populations. Maintains a minimum standard of continuing education for specialty areas. Evaluates and assesses individual staff and nursing student development needs. Counsels and implements a variety of teaching strategies to meet those needs in consultation with nursing management staff. Participates in in-service and other educational programs related to clinical specialties. May identify, develop, and implement nursing research studies on clinical units, in consultation with medical staff consultants. Clinical Management: Consults and coordinates with other health care team members to resolve complex clinical issues and develop short and long-term clinical goals for assigned units. Integrates and evaluates clinical care knowledge specific to the needs of unit individuals; develops unit-specific policies and procedures in consultation with nursing management staff. Identifies and implements corrective actions to improve the quality of individual care; may oversee equipment testing and supplies. Leads and attends various hospital committees. Assists Registered Nurse Supervisor 1 (T&RC) staff with implementing and achieving unit operating changes; may take charge of a unit in the absence of a Registered Nurse Supervisor 1 (T&RC). Resolves individual relation issues. Identifies and reports issues related to individual care, operation, and quality outcomes. Identifies potential conflict situations and facilitates solutions. Registered Nurse Supervisor 1 (T&RC) Plans, directs, and supervises nursing services on an assigned unit, including direct supervision of Registered Nurse 1 (T&RC) and other staff. Directs the development and implementation of nursing care plans on a unit. Provides individual care, and functions as an educator and nursing services expert. Assesses work methods of nursing staff and provides feedback to improve the efficiency and effectiveness of care delivered. Coordinates medical and nursing care needs with other health care providers and departments. Ensures proper functioning of equipment and unit compliance with safety and environmental standards. Monitors nursing care quality standards, and implements corrective actions as needed. Attends and participates in medical committees to represent nursing staff needs and the individual population, and participates in writing of care standards. Identifies staff training needs and collaborates with unit educators to develop and implement educational programs. Job Description The Department of Pediatrics at SUNY Downstate Health Sciences University is seeking a full-time TH Assistant Director of Nursing / Nurse Practitioner. The successful candidate will: Manage and provide care for the patient requiring neonatal intensive care utilizing collaborative consultation with team members, under the direction and supervision (but not necessarily the direct attendance) of an attending neonatologist. Provide prenatal consultations as needed. Receive a distressed neonate in the Delivery Room and initiates appropriate intervention. Perform a complete newborn physical examination and assessment of gestational age, discriminating between normal and abnormal findings. Conduct parent interviews and take family history. Collaborate with neonatologists concerning pertinent historical events, physical findings, diagnostic and therapeutic measures initiated on all admissions. Order appropriate laboratory tests and procedures. Perform diagnostic and therapeutic procedures necessary and appropriate for the care of the patient (from ELBW to LOA babies). Review and provide general care items involved in neonatal care, such as temperature stabilization, hyperalimentation therapy, feeding management, etc. Obtain patient history and performs assessment via observation, interview, and examination of the NICU patient. Order, perform and interpret diagnostic studies. Provide direct treatment and management of health conditions. Document on delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy. Perform procedures, to include intubation, umbilical lines, lumbar puncture, thoracentesis/chest tube placement, PICC line placement, Participate in and attend to high-risk deliveries including stabilization and management of complex critical needs of the newborn. Demonstrate strong newborn assessment, management, and delivery room skills. Participate in neonatal transport, if/when needed.
