Deliver direct patient care, including comprehensive health assessments, physical exams, diagnostic testing, and therapeutic interventions in the home. Develop and implement personalized care plans based on patient needs, preferences, and clinical assessments, collaborating with the IMPACT team. Monitor and evaluate patient progress, adjusting care plans as necessary to address any changes in health status or condition. Administer treatments such as medication management, injections, wound care, and other clinical procedures, following all safety and infection control protocols.
