Job Description JOB DESCRIPTION As a provider assistant at HealthPartners Clinic Stillwater, you are responsible for a variety of patient care duties and other related duties in the clinical setting that include documentation of medical information, administration of medications/vaccinations and treatments, as well as obtaining medical histories, vital signs, etc. The provider assistant assists the healthcare providers in the timely delivery of services per physician/RN direction and according to Clinic policies and procedures. All programs and activities are designed to support the short- and long-term mission, vision, goals, and objectives of HealthPartners Clinic Stillwater. Responsibilities Interview patients and document basic medical history Take vital signs and document findings in the patient record Review annual wellness questionnaire with appropriate patients Administer immunizations Monitor messages and relay clinician responses via phone calls Enter orders for laboratory services Required Qualifications Completion of an accredited Medical Assistant program with or without certification (AAMA or AMT) OR Current LPN (MN and WI license required) OR Graduate of an accredited EMT program plus current EMT license (national and Minnesota) PLUS current CPR certification for health professionals through the American Heart Association or American Red Cross Preferred Qualifications One year medial assistant or LPN experience in a clinic. Experience with Epic electronic medical record software. All candidates should have excellent verbal and written skills; excellent customer services skills; tact and diplomacy in interpersonal interactions with patients, families, and care team members; An ability to demonstrate compassion and caring in all interactions; Understanding of patient education needs, able to effectively share information with patient, family members and care team members as needed; Ability to learn, retain information regarding patient care procedures; Ability to plan, prioritize and complete delegated tasks; Ability to remain calm in stressful situation; Experience with Epic preferred. Job Overview The primary function of the Home Health Infusion Nurse is to provide exceptional in-home IV infusion therapy, nursing care, and response management. In addition, the Home Health Infusion Nurse will administer and monitor patients in the home during treatment and provide the necessary education on treatment regimen, condition management and treatment self-administration when appropriate. Requires the ability to work independently, be detail oriented and to handle multiple tasks in a fast paced environment Responsibilities Deliver care as delegated by physician, providing accurate and clinically appropriate infusion therapy assessments and interventions for all therapies, access devices and disease processes; assessing patient for health status changes, support systems and home environment, updating the care team and documenting in clinical record as indicated. Partner with the patient, family and care team in developing and implementing plan of care with specific goals under the direction of the physician. Initiate and coordinate care conferences with the participation of all members of the care team as needed. Provide education and support to patients and caregivers that enable them to safely and effectively self-administer medications and solutions. This includes storage, aseptic technique, equipment use, side effects and potential complications. Coordinate the discharge of patients when goals are met with the physician and other disciplines to achieve an orderly transition for the patient/family. Description This is a 0.8 FTE working with our Specialty Teams. There are currently no evening, weekend or holiday requirements, but this could change with business needs. This person will work primarily at our St. Croix Falls location but could be asked to travel to all community clinics. Hours are generally Monday - Friday 740-5p with a varying day off. Flexibility is greatly appreciated. The Clinician Technician (CMA/LPN) is responsible for planning and managing patient care according to patient need. Responsibilities also include rooming and interviewing patients, recording medical history and physical condition information including taking and recording patient vital signs. Assesses and evaluates patient’s status by: Writing and initiating plan of care Re-evaluating patient and family/caregiver needs Participating in revising the plan of care as necessary Initiates the plan of care and makes necessary revisions as patient status and needs change. Uses health assessment data to determine nursing diagnosis. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient as appropriate per assessment and plan. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. After hours on-call responsibilities included remaining available for all calls related to patient care and making all appropriate visits as necessary. Remaining with patient and or family through a crisis period until resolved and or relived.
