Job Description The Heart Transplant Nurse Coordinator for the Advanced Heart Failure Comprehensive Care Center provides direct care to patients with multiple, often complicated acute and chronic medical problems. The Nurse Coordinator is the primary point person for the program in the post heart transplant phase of care and works closely with the multidisciplinary team and multiple UCSF departments.
