What You’ll Do Conduct primarily telehealth video visits, with limited and occasional in-home visits to members in your local area. Perform comprehensive assessments and provide evidence-based care focused on prevention, chronic condition support, medication optimization, and health maintenance. Identify and address care gaps, coordinate with external and internal care team members, and contribute to individualized, whole-person care plans. Utilize integrated technology and AI-enabled workflows to reduce administrative burden and maximize meaningful member interaction time. The Infusion for Health Nurse Practitioner Manager will play a crucial role in providing clinical leadership, managing day-to-day operations, and ensuring high-quality patient care within multiple ambulatory infusion centers. POSITION DETAILS Caregiver / CNA (Certification Preferred) Employment Type: Full-Time & Part-Time available Pay Rate: $20-22/hour, depending on experience and certification AVAILABLE SHIFTS Day Shift: 6:00 AM - 2:00 PM Swing Shift: 2:00 PM - 10:00 PM NOC Shift: 10:00 PM - 6:00 AM (Flexible scheduling based on availability and staffing needs) KEY RESPONSIBILITIES Assist residents with activities of daily living (ADLs) Provide compassionate, respectful, and patient-centered care Support resident safety, comfort, and emotional well-being Communicate effectively with residents, families, and care teams Follow care plans and community policies QUALIFICATIONS CNA certification preferred (not required for all caregiver roles) Previous caregiving or senior living experience a plus Dependable, compassionate, and team-oriented WHY JOIN BRIDGEWATER?
