The Nurse Practitioner will participate in structured inpatient sign-out at the beginning and end of shift to ensure continuity of care and will collaborate closely with Trauma surgeons, residents, nursing staff, case management, social work, and ancillary services. The NP II will respond to Rapid Response activations involving Trauma patients, participate in trauma resuscitations as assigned, perform surgical consultations on potential trauma or acute care surgery patients, and assist with bedside procedures within approved privileges and scope of practice (including chest tube removal, wound VAC changes, complex dressing changes, drain removal, laceration repairs, and tracheostomy downsizing or removal). This role includes responsibility for discharge planning and throughput management, including coordination of Durable Medical Equipment (DME), home health services, discharge medications, and removal of barriers to safe discharge. The Nurse Practitioner collaborates with the Trauma Nurse Navigator to ensure appropriate follow-up care and supports departmental patient flow initiatives consistent with operational goals. Working directly under the supervision of the Clinic Nurse Manager, the Licensed Vocational Nurse works with a multi-disciplinary team to provide care coordination and assist with transitional primary care to the unsheltered homeless individuals living in Los Angeles. The ideal candidate must have a commitment to providing high-quality care for complex patients, be dedicated to working and developing a collaborative team model of care and have experience working with both physical and behavioral health diagnoses. The nurse practitioner will be involved in resuscitations of surgical patients from the ED and ICU. Responsibilities include coordination of procedures (including scheduling operations and other procedures, etc) and follow-up, admit and discharge patients (including admission H&P, orders, discharge summary, instructions, and medications) when appropriate.
