Job Description The Nurse Coordinator for the Heart Failure Disease Management portion of the Advanced Heart Failure Comprehensive Care Center coordinates care for our patients being evaluated for general heart failure management, as well as those who may be moved into advanced heart failure therapies workups. This role will coordinate between inpatient and outpatient departments, and manage patient panels including recommending diagnostic testing, lab testing, new and investigational heart failure therapies. This role regularly educates patients on heart failure disease, treatment, therapies, medication indications and the importance of adherence, and more. This role will interface between multiple UCSF departments, review incoming referrals, and coordinate outpatient care for patients with heart failure.This position will rotate through the Diuretic Clinic as needed with responsibilities including initiating IV access, blood draws, administration of IV medications, review of laboratory results, and follow-up patient assessment. Qualifications REQUIRED QUALIFICATIONS 3 years’ ambulatory or acute care RN experience in Cardiovascular nursing including heart failure disease management. Qualifications QUALIFICATIONS/JOB REQUIREMENTS: EDUCATION: ● Graduation from an accredited School of Licensed Vocational Nursing. EXPERIENCE: ● Two years of experience in Subacute preferred.● Previous Long Term Care experience preferred. CERTIFICATIONS/LICENSURE: ● Current California licensure as a Licensed Vocational Nurse.● Current certification in Basic Life Support.● Senior Licensed Vocational Nurses must have successfully completed courses in Intravenous Therapy approved by the State Board, and Pharmacology as recognized by the Board of Vocational Nurses and other unit specific certifications. All Licensed Vocational Nurses must complete IV certification within six (6) months of hire. OTHER SKILLS, ABILITIES & KNOWLEDGE: ● Knowledgeable of the California Nursing Practice Act.● Able to understand and follow all policies, procedures, guidelines, rules and regulations of Nursing Service, AHMC Seton Medical Center, Title XXII.● Computer data entry skills.● Excellent customer service and communication skills.
