The Home Health Travel RN will travel extensively throughout our markets to provide, coordinate, and direct home health nursing care according to physician orders and agency policies. They will perform comprehensive assessments and collaborate with the multi-disciplinary team to develop individualized patient plans of care. Primary Responsibilities: Assume responsibility for assigned patient's care from admission through discharge by effectively executing patient's Plan of Care. Collaborate with the patient, family/caregiver(s), ordering physicians, other care team members, and community resources. Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes. Participate in case conferences with other clinicians by reviewing services, clinical documentation, and discussing pertinent patient care issues and goals. Qualifications: Strong home health knowledge, and recent experience (2 years+) working in the home health industry. In-depth understanding of Medicare Conditions of Participation and payment mechanisms for home health. Strong organizational, planning, problem solving, decision-making, and leadership skills required. Strong interpersonal skills. Ability to communicate effectively in writing and verbally with people who have varied background. Ability to direct and manage a multi-disciplinary team of home health care clinicians, both professional and para-professional. Proof of current CPR certification and valid Driver’s license. Requirements: Registered Nurse with at least 2 years of recent home health experience. Knowledge of HCHB is strongly preferred. Bachelor’s degree in nursing is preferred but not required. License and current registration to practice in related discipline in the states where the company is located – MO, OH, MI, IL, KS Computer skills and ability to use word processing, database, spreadsheet, applications, etc. Ability to travel up to 100% of the time.
