Duties Description Independently performs the duties below: Conducts nursing assessments; and obtains, records, and updates individuals’ health information and vital signs, using standard instruments and procedures. Prepares, implements, and modifies nursing care plans; prepares individuals for and/or assists with examinations and treatments; and administers medications and nursing procedures to individuals, as prescribed Observes, monitors, and records individuals’ symptoms, conditions, and care, including diet and physical activity; and identifies and reports individuals’ responses to medication or changes in health status. Makes clinical rounds to assess individuals’ nursing care needs and observe and participate in the provision of nursing services. Provides health teaching, counseling, and education to individuals, families, and other groups; and develops health improvement programs. Implements infection control protocols and procedures and provides related consultation and instruction to staff. Maintains and updates charts, records, and other documentation, consistent with agency, State, and federal requirements; and may collect data and perform related activities to support medical research. Monitors inventories and ensures that standards for medications, solutions, supplies, and equipment are maintained to meet nursing care needs. Provides clinical supervision and instruction to Graduate Nurses, Licensed Practical Nurses, and direct care staff in completing nursing care tasks. Delegates responsibility for appropriate nursing activities, skills, and/or procedures to nursing and direct care staff; and makes clinical rounds to assess individuals’ nursing care needs and observe and evaluate the provision of nursing services. Acts as liaison to outside agencies and providers to ensure continuity of care and the necessary follow-up for individuals; advocates for individuals’ needs; and may provide instruction to providers on individuals’ discharge plans. May perform the full range of supervisory duties over Graduate Nurses, Licensed Practical Nurses, and direct care staff; provide information to Registered Nurse Supervisors; and prepare or contribute to the preparation of reports and performance evaluations for lower-level staff involved in nursing care activities. Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title. At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. Geographic Pay Differential is added to the job rate ($85,138). Description RN/Case Manager MSH Case Management FT Weekdays/Weekends/Holidays The Case Manager (CM) will be responsible for all aspects of case management for an assigned group of patients to determine the appropriateness of the Level of Care, assist in the development of the plan of care; ensure that the plan is implemented in a timely basis and identify the expected length of stay (ELOS). The CM works collaboratively with physicians, social workers, clinical nurses, home care services, and other members of the interdisciplinary team as needed. The CM actively participates in specific clinical initiatives focused on reducing the length of stay (LOS), improved efficiency, quality and resource utilization. Assignment will be by units/clinical areas of practice and may require responsibility on other units/services Responsibilities 1. Principle Duties and responsibilities include, but are not limited to: a. Reviews all new admissions to identify patients where utilization review, discharge planning, and/or case management will be needed using standardized criteria to achieve optimal patient outcomes and appropriate reimbursement for the organization. b. Performs continued stay reviews utilizing standardized criteria to justify continued inpatient stay. c. Collaborates with Physician and other clinicians to expedite diagnostic testing, treatment and consultations. d. Documents all clinical reviews in Case Management Documentation system. e. Supports the mission, vision, philosophy and goals of the Medical Center. 2. Case Management: The CM process will include: a. Assessment of the patients clinical, psychosocial, and functional status in collaboration with the interdisciplinary team. b. Identification and documentation of variances affecting the LOS and the discharge planning process. c. Conducts follow-up of any delays in treatment or reporting of results. d. Planning/developing specific goals with the physician, interdisciplinary team, and the patient and/or family. e. Implementation and coordination of specific activities, strategies, and interventions to move the patient through the continuum of care. f. Documentation of outcomes achieved and identified internal and external barriers. g. Identifies reasons for readmissions and collaborates with interdisciplinary team on strategies to reduce readmission rate. h. Appropriately identifies and refers cases to the physician advisor to support timely progression of patients along the continuum of care and (appropriate) discharge planning. i. Interacts with patient/family to discuss plan of care and coordination of services based on clinical needs and available resources. 3. Utilization Review: a. Maintains a working knowledge of the UR (Utilization Review) requirements of each payor within the patient population; b. Provides the clinical information requested by the managed care companies as part of the concurrent review in a timely fashion. c. Provides clinical information requested by the managed care companies in accordance with contractual agreements. d. Works collaboratively with physicians and managed care companies on concurrent denial appeals e. Communicates clinical information to the payor, as needed, coordinating direct communication between physician and payor Medical Director as required. f. In psychiatry, the CM will document UR notes in the medical record as per OMH guidelines. 4. Discharge Planning: a. Responsible for assessment, communication and monitoring of discharge planning process (The clinical nurse initiates the discharge planning process on admission). b. Obtains authorizations from managed care companies for post-discharge services. c. Assesses for clinical readiness and completes the Hospital and Community Patient Review Instrument (PRI) for patients requiring Residential Health Care Facility placement. d. Liaison with financial department for current insurance coverage. 5. Collaborates and participates in the appeals process with all members of the interdisciplinary team. 6. Collaborative Relationships: a. Develops and maintains effective working relationships with interdisciplinary team and Case Managers within the managed care organizations. 7. Participates in nursing orientation. 8. Interfaces with other departments in the Medical Center. Qualifications Education Requirements Bachelors Degree in Nursing; Masters preferred. Experience Requirements Previous experience as in homecare, long term care or utilization review preferred. Discharge Planner or Case Manager preferred. Patient Experience Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions but not limited to: Admission Welcome, Bedside Shift Report, HELP, AM/PM Care, Dedicated Sit Down, Purposeful Hourly Rounding (PHR)/Care Checks, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration as applies to the care setting (inpatient, procedural ambulatory). Participates and facilitates Communication Huddle with staff at the start of the shift, identifying high-risk patients who require special attention and escalates concerns in a timely manner for resolution.. Communicates to nursing leadership concerns or issues identified during huddle and throughout shift. Initiates service recovery for patients and family members who have concerns and escalates to Nursing Director/designee and Patient Relations as needed. Interprets and communicates patient experience scores and benchmarks and keeps staff informed of progress and areas of opportunity. Empowers staff to use evidence-based practices to increase understanding of patients’ perceptions of care. Demonstrates caring, respect, compassion, empathy and active listening through verbal and body language and actions. Quality and Safety Designs, supports and leads a safe, therapeutic and efficient patient-centered care environment for professional practice. Identifies and investigates issues or variances in practice/operations by participating in the RCA process and implements corrective action plans/safety solutions accordingly. Participates in unit and hospital based quality, patient safety and patient experience process improvement initiatives Utilizes the principles of Lean methodology for process improvement through the use of the Daily Management Boards. Fosters a fair and open Just Culture that encourages error and near-miss occurrence reporting. Leads interdisciplinary quality improvement teams using the principles of high reliability. Reviews unit progress, changes, and compliance with quality and safety metrics with clinical nurses and other members of the healthcare team. Responds to escalations of potential safety hazards or gaps from best practice by correcting concerns following institutional chain of command protocol. Monitors institutional information technology infrastructure and corrects gaps in clinical nursing documentation. Leads staff participation in quality and safety initiatives and mandated compliance measures. Demonstrates accountability for all unit quality and safety practices and compliance, and ensures required evaluation of staff competencies is maintained. Operations and administration Supports implementation of the mission, vision, goals and objectives of the department, throughout the clinical service and individual patient care units. Provides administrative and clinical oversight to ensure that all activity (staffing, support services, inter disciplinary collaboration, resources and financial viability) supports the goals of the organization. Works collaboratively with physician dyads and all members of the healthcare team to promote patient care and regulatory standards. Prioritizes recruitment and retention strategies in accordance with system and local hospital retention goals. Maintains a nursing unit in compliance with all regulatory agency mandates, including the regulations of NYS DOH and The Joint Commission. Oversees 24 hour patient care delivery, throughput, staffing and processes to ensure efficient and effective operations in all clinical specialties. Implements and monitors unit salary and supply budgets, including capital equipment allocation, budget variances, management of OT and vacancies according to MSHS financial goals. Monitors availability and maintenance of nursing equipment and supplies Maintains position control of established positions and manages the recruitment and retention of qualified staff. Maintains patient/employee confidentiality in the management of information. Evaluates nursing staff performance and mentors, coaches, counsels and disciplines staff as needed. Evaluates all newly hired/transferred staff and participates in the decision about passing the probationary period. Participates in the ongoing and completes the annual performance appraisals for all staff. Completes performance improvement plans of employees as needed and shares feedback with the Associate Directors/Directors and Senior Directors accordingly. Interprets and implements the associated collective bargaining agreements and System and Hospital policies and procedures. Performs disciplinary actions as necessary with collaboration with Nursing and Labor Relations. Acts as a management witness in disciplinary activities. Insures all staff have met their orientation goals/weekly progress reports and annual competency requirements and participates in the Competency Planning Tool for unit/service. Conducts regular staff meetings to ensure communication with staff about progress, changes and compliance with all hospital policies and procedures. Maintains records of minutes for staff team meetings. Encourages, recognizes and rewards staff participation in quality and safety initiatives, compliance and successes. Facilitates organizational change initiatives by using effective strategies. Leads a shared decision making model by establishing a structure of shared governance, just culture and staff engagement in practice councils, departmental meetings and initiatives. Responds to protest of assignment in a timely manner as detailed in the collective bargaining agreement and discusses the resolution in unit staff meetings. Participates in staffing planning committees to meet all NYS mandatory requirements to insure safe staffing Professional Development Performs as a clinical leader who actively creates and supports nursing practice and an environment of care that reflects clinical excellence. Organizes the environment of care to support clinical colleagues, patients and their families and members of the healthcare team. Projects and cultivates a professional image to colleagues by communicating caring, respect, compassion, empathy and trust. Incorporates ethical principles into decision making for patient and family in collaboration with the clinical nursing staff and the healthcare team. Qualifications Education Requirements Bachelor of Science with a major in nursing and master?s degree in nursing or healthcare-related field or be enrolled in a program preferred Experience Requirements Minimum of 3-5 years of progressive clinical and nursing expertise on a clinical psychiatry unit and Med/Surg unit 3-5 years of progressive management/leadership experience/success required.
