Now Hiring - Certified Home Health Aide (CHHA) - North Team This Certified Home Health Aide will be responsible for, but not limited to: Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails. Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient. Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse. Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry. Feeds and assists with meals. Obtains temperature, pulse and respirations as supervised by the Hospice Nurse. Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information. Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies. Performs grade I-IV Decubitus Care; performs preventive skin care measures. Applies simple dressings, maintaining principles of aseptic technique. Applies warm and cold compresses, consistent with facility procedure. Performs irrigations, consistent with facility procedure. Performs other procedures related to skin care, as necessary. Administers medications correctly and safely; correctly identifies medication in terms of action, dosage, side effects, and implications for the patient. Meets standard on medication administration examination. Demonstrates preparation of local solutions. Administers and documents medication correctly. Educates patients and significant others related to drug and food interactions. Participates in patient and family education. Provides patient with an explanation of his/her condition as indicated. Communicates assessment data in an orderly fashion by recording, updating and verbalizing pertinent information to care team members and to appropriate agencies. Recognizes and utilizes health teaching opportunities and resources /materials available for this teaching. Provides for early discharge planning and appropriate referrals for post-hospital care. Evaluates effectiveness of teaching by feedback from patient/family and documents same. Performs related duties as required. Job Qualification Graduate from an accredited School of Nursing, required. Bachelor’s Degree in Nursing, preferred. Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date. Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed. Valid Driver's License in New York State required may be required depending on area of assignment.
