JOB DESCRIPTION SUMMARY The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Provides health care instructions to the patient as appropriate per assessment and plan. 10. Supervises ancillary personnel and delegates responsibilities when required. 4. Your Job The Registered Nurse will be responsible for the administration and management of Nursing Services to residents in accordance with orders of the physicians and total needs of the residents. The Registered Nurse will be responsible for the 24-hour supervision of Nursing Services and directs the Nursing Department to maintain quality standards of care in accordance with the Federal, State and company standards as well as guidelines and regulations. In the absence of the Administrator, The Director of Nursing will assume the responsibility for the center's operations. This position will conduct the nursing process – assessment, planning, implementation and evaluation – under the scope of the State’s Nurse Practice Act of Registered Nurse licensure. Principal Responsibilities Assumes ultimate responsibility for coordinating plans for the total care of each resident which comply with physician’s orders, governmental regulations and facility resident care policies. Communicates and interprets policies and procedures to nursing staff, and monitors staff practices and implementation. Participates in all admissions decisions, and may visit prospective residents before admissions. Participates in daily and weekly management team meetings to discuss resident status, census changes, personnel, or resident complaints or concerns. Other duties, responsibilities and activities may change or assigned at any time with or without notice. Qualifications Currently licensed as RN in the state practiced. Associate or Bachelor Degree from an accredited nursing school required. Five years in long-term or acute health care preferred; At least 2 years nursing supervisory experience required.

The Medical Assistant administers and performs triage and direct care to assigned patients under the direction and supervision of the RN/Nurse Manager. This includes functions such as height/weight and vital signs, updating medical records, as well as assisting the clinicians with exams as requested. Also ensures a smooth flow of patients through the office by providing efficient basic care to patients. The Medical Assistant performs routine administrative and clinical tasks to keep the office running smoothly. The Medical Assistant also performs secretarial support for the office, assist the front desk operations in performing general clerical tasks, such as checking patients in and out, filing, copying, answering the phone, and related duties as requested by the Practice Manager.
