Demonstrates knowledge, skills, and techniques necessary to care for residents with the following needs: therapeutic, neurodermitis, dialysis, IV therapy, infectious disease, and wound care if applicable. 3. Provides nursing care in accordance with Resident Care Policies and Procedures and ensures the safety and well-being of the residents is maintained. 4. Attends all mandatory in-services by the employee's anniversary date. 5. Maintains and updates New York State license incompliance with New York State regulations. to meet resident and facility needs. 9. Follows established philosophy and objectives of the Nursing Service Department to ensure quality of resident care is established. 2. Follows established Policy and Procedures and offers input to implement change when necessary. 3. Demonstrates accountability for the maintenance of resident care as defined by the high quality standards of the facility. 4. Makes daily rounds in all areas of nursing care under his/her supervision, visiting each unit to evaluate overall unit performance, and to receive comments relating to the residents needs and problems, and implement necessary changes. 5. Delegates responsibility to nursing personnel through work assignments, etc. for direct nursing and non-nursing care of patients during the shift. 6. Supervises and evaluates the performance of all nursing personnel under his/her direct supervision. 9. Completes appropriate evaluation summaries and assists with those completed by Nurse Manager and/or Charge Nurses, as needed. 11. Assesses, monitors and treat specific patient care conditions pursuant to physician orders. I5. Maintains open line of communication with attending physicians regarding patient care and/or treatment issues. 16. Demonstrates ability to act as the Assistant Director and/or Director of Nursing in their absence. 17 Ensures the coordination of nursing care with other departments and disciplines within the facility to best meet resident care objectives. 18. Conducts periodic rounds to observe facility appearance and safety, and makes recommendations assignment for nursing personnel in regard to the concerns and/or report these concerns the appropriate personnel. C. Role Responsibilities-Nursing Systems: l. Ensures that systems are established and consistent throughout facility. These include but are not limited to (as per standardized procedure): a) Develops unit management systems for group assignments and Nursing Assistant responsibilities. d) Ensures turning and positioning schedules are followed every two hours(or per Care Plan). Ensures turning and turning and positioning system can be easily monitored. e) Ensures that resident ambulation occurs as an observed system (i.e. before the meal period in the dining room). Q Oversees scheduling of nurses for employee meals or breaks, ensuring that breaks are not taken while residents are dining. g) Conducts periodic reviews to ensure supplements are passed according to the developed schedule. h) Conducts periodic review to ensure that the environment is clean, orderly, and organized. Ensures that cleaning schedules are in writing and audited daily. Reviews and implements infection control and exposure control plans according to Resident Care Policies and Procedures. i) Ensure completion of Change of Condition/24-HourReport daily and presents report at A.M. Interfacing Meeting. j) Demonstrates ability to perform all physical and mental job functions of a Certified Nursing Assistant, Licensed Practical Nurse, and Registered Nurse. D. RoleResponsibilities- Nursing Documentation: 1) Ensures that nursing documentation meets the following standards: a) Care Plans match monthly summaries, Nursing Assistant Care Sheets, Nursing Assistant Worksheets, Treatment Sheets and Physician Orders. b) Maintains and evaluates statistics for pressure sores, resident Accidents and Incidents, number N/G or G tubes, number of justified foley catheters, resident census, resident and employee infections. and updated by the Charge Nurse changes occur. e) Ensures all communication to nursing by other disciplines are flagged and the Charge Nurse is responsible for implementing the changes on all forms. Ensures no medications are left unattended. d) Assesses effectiveness and observes for interaction and reaction of medications and treatment. e) Demonstrates knowledge of residents’ allergies according to established Care Plan. f) Documents medication and treatment administration in a timely and accurate manner according to Resident Care Policies and Procedures. g) Orders appropriate medications from pharmacy as needed in accordance with Resident Care Policies and Procedures. A Hospice Inpatient Unit Licensed Practical Nurse (LPN) is responsible for carrying out patient care in accordance with physician's orders under the guidance and direction of the Registered Nurse in charge and supervision of the Team Manager. Essential Duties and Responsibilities for a Hospice Inpatient Unit Licensed Practical Nurse (LPN): Provides skilled nursing care to patients as directed by the IDG plan of care in accordance with agency policies. This includes personal care, vital signs, and administration of medications, treatments and any other procedures within their professional practice following the policies of the agency. Documents to agency standards, timely completion, timely signatures, using A-I-R protocol. Reinforces patient and family teaching that may include information on the disease process, self-care techniques, and prevention strategies under the guidance of registered nurse assigned to the same case. Helps to achieve and maintain continuity of patient care by assisting in planning and exchanging information with other health professionals. Assists the physician and registered nurse in performing specialized procedures. Elicit patient/family complaints or unusual occurrences (i.e., reportable incidents). Demonstrates proper sharps disposal, hand hygiene; waste disposal; use of personal protective equipment. Communicates current health status and needs of patients to appropriate health professionals, contributing to the coordination of the total plan of care inclusive of review/revision of the nursing care plan, evaluation of the effectiveness and attainment of patient care outcomes, and discharge planning. Participates in the quality improvement program and documents quality assurance data as required. Participates in weekend, holiday rotation and back up visits as needed. Adheres to requirements of the organizations policies and procedures that are both patient focused (clinical) and organizational focused (administrative). Adheres to HIPPA regulations and policies. Maintains the security of patient electronic/written records.
