Key Responsibilities · Conduct psychiatric and physical health assessments for program participants. · Develop and coordinate care plans with the multidisciplinary treatment team. · Monitor participant health status and modify treatment plans as needed. · Administer medications, injections, vaccines, and perform phlebotomy as ordered. · Monitor vital signs and medication side effects and communicate findings to the treatment team. · Coordinate medical appointments and collaborate with psychiatrists and community physicians. · Provide treatment, rehabilitation, and recovery support services. · Advocate for participants and assist them in accessing community resources and services. · Maintain accurate clinical documentation, progress notes, and medical records. · Participate in team meetings, supervision, and required trainings. · Provide crisis intervention services on a rotating basis. Qualifications · Currently licensed as a Registered Professional Nurse (RN) by the New York State Education Department. · Minimum two (2) years of experience working as a Registered Nurse. · Minimum two (2) years of experience working with individuals with mental health conditions in a health or social services setting. · Strong written, verbal, and interpersonal communication skills. · Ability to assess situations, synthesize information, and make appropriate clinical decisions. · Eligible for full and unconditional participation in Medicaid and Medicare programs. · Computer proficiency including MS Word, Excel, PowerPoint, and electronic health systems. · Comfort providing services in community settings such as shelters, public spaces, and outreach environments. · Ability to travel within the community, including use of public transportation. · Bilingual Spanish preferred. · Must be fingerprinted and cleared by the New York State Justice Center.
