The Clinical Assessor / Coordinator, LPN identifies actual and potential mental and physical health problems and accurately determines the necessary classification and regimes to assist with the appropriate placement of a potential admission to the Gurwin Jewish Health Care System. HOURS: Monday through Friday 9:00am-5:00pm EDUCATION AND EXPERIENCE REQUIREMENTS Licensed to practice as LPN in New York State. Minimum of one year experience working in a hospital, long term care or certified home health agency (CHHA) DUTIES : Demonstrates knowledge and the capacity to accurately interpret the PRI/RUG Classification System. Properly manage referral case load and respond in a timely manner via Navi/Allscripts and eFax. Accurately assesses the classification and patient disposition upon screening. Ability to coordinate referral via EMR including verification of benefits Ability to coordinate the bed board/census when needed. Ability to assess potential patients at partnering hospitals. Addresses /accommodates the age-specific physical, psychosocial and emotional changes in residents/patients when planning programs and/or delivering care. Maintains updated knowledge and understanding of the comprehensive services available for new admissions. Discusses/interprets patient's potential and present status with appropriate disciplines. Demonstrates the ability to establish and maintain a professional rapport with members of the health care team in facilities and referring hospitals. Demonstrates a high level of communications skills and interviewing skills with patients, families, and all disciplines. Communicates clearly and comprehensible to residents, patients, and family members, understanding changes in cognition an allowing sufficient time for the processing of information. Demonstrates appropriate understanding of the needs and behaviors of elderly residents/patients as they differ from other age groups. Demonstrates the ability to discern the needs and behaviors of sub-acute/rehab patients appropriately, as they differ from those of the elderly. JOB SUMMARY: Assumes responsibility for the planning, organizing, implantation and evaluating programs of Medicare Assessments and Minimum Data Set Plus Coordinator. Functions as the Employee Health Nurse for the Brooklyn Gardens Nursing and Rehabilitation Center. Performance Indicators 1. Ensures all Medicare determinations are completed in a timely manner. 2. Informs the Director of Nursing, Business Office and Social Service of all Medicare Determinations and Discontinuation of coverage. 3. Assess that necessary documentation required for Medicare coverage is completed. 4. Responsible for completion of Certification and Recertification forms and skilled care forms. 5. Coordinates the MDS+ form with the Unit Coordinator and related disciplines. 6. Delegates responsibilities for completing sections of the MDS+ to staff that has clinical knowledge about the resident. 7. Establish the facility’s policies and procedures to assure that key clinical personnel on all shifts are knowledgeable about the information found on the resident’s MDS+ and report changes on the resident’s status. 8. Coordinates with the Unit Coordinator and other disciplines the resident assessment (MDS+) and the care planning practices (RAPS). 9. Participates in the Interdisciplinary Resident Care Plan meeting to ensure that the activities of the committee result in proper identification of problems, complications, and risk factors. 10. Coordinates with the Director of Nursing the quarterly preparation and submission of PRI’s for RUG reimbursements. 11. Participates in departmental and interdisciplinary committees which influence or determining policies affecting nursing practice. 12. Coordinates the Employee Health Services in the Nursing Home. 13.Follows all appropriate safety procedures. 14. Respects residents’ rights at all time. 15. Performs all related duties as required and unrelated duties as requested during emergencies.

These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit.
