Overview: As a Medical Assistant at Hometown Urgent Care, you will play a crucial role in supporting healthcare delivery by assisting healthcare professionals in various clinical and administrative tasks. Your contribution will be vital in ensuring the safety, welfare, and comfort of our patients. *This position is about 2 week days per week plus every other weekend. This cannot be weekend only. Duties: Responsible for taking patient history and obtain vital signs Perform basic testing or screenings such as: vision, audio, urine drug screens, breath alcohol, EKGs, pulmonary function testing. Perform necessary phlebotomy for collection of laboratory samples. Accurately complete corresponding paperwork/computer work for lab tests ordered on blood, urine, tissue, and culture samples. Able to perform subcutaneous, intradermal, and intramuscular injections and accurately complete paperwork according to policy for that procedure. Perform point of care testing such as: strep, mono, glucose, influenza, Urine pregnancy, Urinalysis, blood glucose levels. Dispense and document medications and other treatment modalities. Insure sterile technique, instrument cleaning and sterilization, proper use of the autoclave. Schedule and coordinate necessary records for ancillary care for patients. Assist providers with exams and testing (pelvic exams, eye irrigations and ear irrigations, I & D’s and splinting/ ortho-glass preparation Perform occupational medicine testing such as: urine drug screens, breath alcohol testing, audiogram, and DOT physicals Maintain confidentiality with all patient information (HIPAA). Ensures equipment is in working order. Reports equipment malfunctions to manager. Logs laboratory procedures completed. Processes related paperwork using computer equipment as directed. Responsible for patient call backs, scheduling physician referrals; specialist’s referrals and pharmacy issues; and patient’s education. Maintains examination rooms, sterilization of equipment and stocks necessary medical supplies. Performs duties to ensure good patient flow. Registration duties: explain to patients' accepted insurances and those out of network and knowledgeable of all pricing. Properly collect and record payments, complete registration screen in a timely manner. Travel to care centers within designated region Protects patients' rights by maintaining confidentiality of personal information Cleans and stocks the site with other members of the team Training other team members. Additional job responsibilities may be assigned. 1-2 years previous experience working in a physician practice or clinic setting is preferred. Basic computer skills, including the use of Electronic Medical Records (EMR) Ability to function independently without close supervision, and to consistently exercise discretion and the highest level of good clinical and other professional judgment. KEY RESPONSIBILITIES Oversees the overall care of the patient from beginning to end of episode Updates all doctor orders in patient chart accordingly Notifies doctor of any significant changes of the patient’s condition Manages multi-disciplinary care as applicable Provides skilled nursing care in a patient’s home as ordered by the attending physician Observes and monitors patient conditions Performs OASIS assessments to develop an individualized plan of care and makes adjustments as needs change Administers medication as prescribed by the physician Helps decrease re-hospitalizations by front loading visits for high risk patients Promotes continuity of care with appropriate admissions, transfers and discharges Counsels patient and family on the disease/injury processes and how to manage Incorporates patient and family in development of plan of care Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions Listens to patient and family members Oversees and supervises total care of patient provided by nurse aides and LPNs Evaluates the treatments and medical condition of the patient according to the plan of care Determines if the level of care being provided meets the patients’ needs Communicates with the LPNs and nurse aides on supervisory visit results Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines Documents all aspects of treatment, assessments, and patient education Maintains active RN license Communicates with scheduler any changes outside the normal Participates in all on-call requirements and case conferences Attends in-service trainings and mandatory agency meetings Drives to patient’s primary location per scheduled visit. Daily attendance at assigned visit locations. Documents all aspects of subsequent, discharge, eval/recert/resumption of care visits within 24 hours of visits. Documents all aspects of start of care visits within 24 hours of visits. Completes and submits all required documentation within specified company requirements. Follows plan of care as permitted within the scope of practice for a Registered Nurse. Fulfills RN visits in assigned geographic location per patient need within federal and state specifications. Other duties as assigned. QUALIFICATIONS Minimum of one year of experience as an RN in an acute care setting. Home care experience is desired. Job Description Registered Nurse (RN -CN 1), Night Shift, Surgical Intensive Care Unit (SICU) ONA 2 CCP- Surgical/Trauma Intensive Care Unit (SICU) – 34-bed unit The Surgical/Trauma Intensive Care Unit is a 34-bed unit specializing in the complex care of adult patients requiring surgical interventions. The major specialties treated in this ICU are Trauma Surgery, Transplant Surgery, Acute Care Surgery, and Oncologic Surgery. As the region’s only adult Level 1 trauma center, the SICU is always ready to receive any critical trauma patient requiring resuscitation and specialized nursing care. We have a variety of solid organ transplant recipients, including liver, kidney-pancreas, and kidney transplants. We care for complex oncologic, hepatobiliary, and general surgery patients who are referred from across the country. The majority of our patient population is in critical condition, with most patients generally stabilizing within 24-48 hours following admission. Patient care focuses on stabilizing cardio-pulmonary functioning, maintaining adequate circulatory volume and perfusion, preserving the patient’s protective mechanisms, and controlling pain. Our fast-paced SICU receives 5-8 admissions daily from the OR and ER. This requires a multidisciplinary team rounding process to ensure patients are ready to transition to the next phase of care on the surgical floors. The SICU nurse plays a vital role in this process. Evaluation – Continuously evaluates nursing practice related to the plan of care in relation to the standards of care established through evidence-based literature and standards established by professional organization that have been built into order sets in the electronic medical record, the individual plan of care, and patient outcomes and goals. Maintains continuity of care by continuous evaluation during ”handoff” report, rounding, and upon transfers to other specialty units and post care facilities.
