The Hospice Team Nurse completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 3. Communicates with community health related persons to coordinate the care plan. POSITION QUALIFICATIONS 1. Graduate of a National League of Nursing accredited school of nursing. 2. Current licensure in State of CA and CPR certification. 3. Registered nurses shall have a minimum of one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN license. Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Management experience not required. Responsible for supervising hospice aides. 5. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. 6. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. 7. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

PEDIATRIC HOSPICE RN CASE MANAGER JOB DESCRIPTION Overview The Hospice Pediatric RN Case Manager will work with our Pediatric team. Primarily responsible for providing professional nursing care to assigned patients. The Pediatric Intensive Care Unit (PICU) at UCSF Benioff Children's Hospital provides extraordinary high-level care for children who require continuous monitoring. The unit has 20 beds for infants and children suffering from a variety of conditions or recovering from surgery. More than 1,000 children per year are admitted into the intensive care units at UCSF Benioff Children's Hospital arriving via ambulance, helicopter and airplane from throughout the western United States. The Pediatric Transitional Care Unit (TCUP) is a 18-bed unit for infants and children suffering from a variety of conditions or recovering from surgery. The TCU provides care to patients with a moderate to high complexity of disease such as those recovering from solid organ transplantation, titration of anticoagulant therapy and other infusion, and post-operative specialty surgery care (ENT, neurosurgery). Patients requiring respiratory support including BIPAP, CPAP, high flow nasal cannula and patients requiring mechanical ventilation with tracheostomy in place are also routinely admitted to the TCUP. The Nurse Practitioner will act as a full member of the team taking responsibility for the safe delivery of care in collaboration with attending physicians, fellows and residents. This team member will meet productivity expectations as set forth by the Division Chief, performing any duties related to patient care activities that are appropriate and clearly delineated. Qualifications Required Qualifications: Graduation from a California Board of Registered Nursing recognized Nurse Practitioner Program with a master’s or doctoral degree aligned with an Acute Care Pediatric Nurse Specialization Nurse Practitioner 1: Minimum of two (2) years of recent experience working as an RN in a Pediatric ICU setting Nurse Practitioner 2: Minimum of two (2) years Nurse Practitioner experience in a pediatric inpatient or emergency department setting AND minimum of two (2) years inpatient RN experience in pediatric critical or transitional/intermediate care OR Minimum of 1 year of NP Experience in Pediatric Critical Care OR Completion of a Pediatric Critical Care APP Fellowship Program Preferred Qualifications Minimum of three (3) years of experience working as an AC-PNP in a level III Pediatric Intensive Care Unit Licenses/Certifications California Registered Nurse (RN) License without restriction California Nurse Practitioner (NP) License without restriction California Nurse Practitioner Furnishing Number NP national board certification from an accepted certifying agency aligned with the patient population and clinical role - Pediatric Inpatient (CPNP-AC).
