Certified Nursing Assistant Summary/Objective In keeping with our organization’s goal of improving the lives of the Guests we serve, the Certified Nursing Assistant (C.N.A.) plays a critical role in providing superior customer service and nursing care to all Guests. Provides quality nursing care to Guests in an environment that promotes their rights, dignity and freedom of choice. 2. Carry out assignments required for the Guest’s activities of daily living (ADL’s) which include but not limited to bathing, dressing, grooming, toileting, and feeding. 4. Attends to individual needs of all Guests in regards to incontinent care, transferring, ambulation, range of motion, communication and other needs. 5. Provides care that maintains each Guest’s skin integrity to prevent pressure ulcers, skin tears and other damage by changing incontinent Guests, turning, repositioning immobile Guests and by applying moisturizers to fragile skin and other areas. 6. Performs various tasks assigned by the floor nurse including checking vital signs, weighing Guests, applying creams/ointments and collecting specimens. 7. Must be knowledgeable of individual care plans and support the care planning process by providing supervisors with specific information and observations of the Guest’s needs, preferences and report any behavioral changes. 8. Assists the Wound Care Nurse as directed. 9. Completes medical records documenting care provided and other information in accordance with nursing policies while maintaining strict confidentiality. 10. Must answer and respond to call lights promptly and courteously. 11. Assist with new admissions, re-admissions and assist with the transfer of Guests to different rooms within the facility. 12. Communicates and interacts effectively and tactfully with Guests, visitors, families, peers and supervisors. 13. Attend and participate in departmental meetings and in-services as directed. Required Education and Experience • High school diploma or equivalent. • Valid, unencumbered Nursing Assistant Certification (C.N.A.) in the state of practice. • C.P.R. Certified where required in the state of practice. Responsibilities The Registered Nurse is responsible for the delivery of safe patient care utilizing the nursing process of assessment, planning, implementation and evaluation. Provides direct patient care within the scope of practice. The Registered Nurse also directs and guides patient/family teaching and activities of other nursing personnel while maintaining standards of care in the ICU. We're looking for nurse practitioners in the Chicago suburbs (Joliet-Palatine) area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits. You'll be trained and supported by our Chief Medical Officer, a board-certified geriatrician who designed the program's clinical content. Your first site day is run alongside a member of our team. Schedule Site days are on weekdays. The presentation is the fixed part of the day. The rest depends on how many residents sign up. Expect roughly 4 to 8 hours per site day, a few times a month to start, with hours growing as bookings grow. What you'll do Conduct 45 to 60 minute longevity visits built on a comprehensive geriatric assessment: what matters most to the resident, medication review and deprescribing, cognition (including the Mini-Cog), mood, sleep, nutrition, mobility, fall risk, daily function and advance care planning Evaluate eligible residents for weight management, including GLP-1 therapy where clinically appropriate Identify residents who would benefit from follow-up care, such as a fuller cognitive review, ongoing medical follow-up, psychiatry or therapy, and coordinate with their primary care provider Document every encounter accurately and completely, to Medicare standards, in our EHR Represent Total Life professionally with residents, families and community staff Travel to assigned senior living communities in the Chicago suburbs (Joliet-Palatine) area Requirements Active, unrestricted Illinois APRN license as a nurse practitioner and Illinois RN license from the Illinois Department of Financial and Professional Regulation (Illinois is not a Nurse Licensure Compact state, so a multistate RN license does not apply) Current national NP board certification Active DEA registration, an Illinois mid-level practitioner controlled substance license from IDFPR, and registration with the Illinois Prescription Monitoring Program Ability to practice under a written collaborative agreement with a physician, as required in Illinois, unless you hold Illinois Full Practice Authority (APRN-FPA) Clinical experience caring for adults 65+ Experience with Medicare documentation and E/M coding Your own malpractice insurance (minimum $1M per occurrence, $3M aggregate) that covers in-person care, not just telehealth, or the ability to obtain it before your first site day Comfort presenting to groups and speaking with older adults and their families A valid driver's license and reliable transportation Ability to complete each community's onboarding requirements, which may include a background check, TB screening and immunizations Weekday availability for on-site days in the Chicago suburbs (Joliet-Palatine) area, with hours confirmed in advance Nice to have Experience with comprehensive geriatric assessment Familiarity with cognitive screening and staging tools (Mini-Cog, MoCA, FAST); MoCA certification is a plus Experience in senior living, long-term care, SNF, hospice, palliative or house-call settings Experience prescribing GLP-1s for weight management Telehealth experience Licenses in more than one state Adult-Gerontology NP certification (AGNP/AGPCNP), or Family NP with a geriatric focus or training What we offer Mileage reimbursement for site days Training from our CMO in geriatric assessment, cognitive tools (Mini-Cog, MoCA, FAST) and our documentation Presentation materials, resident sign-ups and scheduling handled by our team Flexible contractor schedule, with hours that grow as your calendar fills The chance to help build a program for a population the longevity field has overlooked Total Life welcomes applicants of all backgrounds and is committed to equal opportunity.

Provides general nursing care, such as positioning residents, transferring, lifting and turning residents, applying/utilizing special equipment, assisting in use of bedpan or commode and ambulating the residents. Performs all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors. Supports and maintains a culture of safety and quality. Takes and records temperature, pulse, respiration, weight, blood pressure and intake-output. Makes rounds with outgoing shift; knows whereabouts of assigned residents. Makes rounds with oncoming shift to ensure the unit is left in good condition. Adheres to policies and procedures of the facility and the Nursing Department. Participates in socialization activities on the unit. Ability to communicate with the cognitively impaired. Turns and positions residents as ordered and/or as needed, making sure no rough surfaces are in direct contact with the body. Lifts and turns with proper and safe body mechanics and with available resources. Follows skin care program and takes preventative measures. Checks for reddened areas or skin breakdown and reports to nurse. Ensures residents are dressed properly and assists, as necessary. Ensures that used clothing is properly stored in bedside stand or on hangers in closet. Ensures that all residents are clean and dry at all times. Checks unit for adequate linen. Folds neatly and arranges linen in linen closet. Cleans linen cart. Provides clean linen and clothing. Makes beds. Treats residents and their families with respect and dignity. Restrains residents properly, when ordered. Accompanies residents to appointments, as directed. Provides reality orientation in daily care as needed. Understands stages of dementia. Prepares residents for meals, serves and removes food trays and assists with meals or feeds residents, as necessary. Monitors nutritional intake. Distributes drinking water and other nourishments to residents. Monitors intake. Knowledgeable about the types of isolation, performs general care activities for residents in isolation. Answers residents' call lights, anticipates residents' needs and makes rounds to assigned residents. Assists residents with handling and care of clothing and other personal property (including dentures, glasses, contact lenses, hearing aids and prosthetic devices). Transports residents to and from various departments as requested. Reports and, when appropriate, records any changes observed in condition or behavior of residents and unusual incidents. Completes STOP & WATCH. Participates in and contributes to interdisciplinary care conferences. JOB SUMMARY Responsible to provide direct nursing care to the customer, and to supervise the day-to- day nursing activities performed by the nursing assistants. Place orders for medications and treatments as necessary following established budgetary guidelines. Utilize PCC 24-hour summary concerning recommendations/concerns related to customer care needs. Fill out complete accident/incident reports. Encourage attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies. Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the customer, as well as the customer’s response to the care. Complete required assessments upon the customer’s admission, transfer, and/or discharge. Receive telephone orders from physicians and record on the Physicians’ Order Form and P.O.S. Record new/changed diet orders. Forward information to the Dietary Department. Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, care of the dead/dying, etc., as required. Participate in the orientation of new customers/family members to the facility. Arrange for diagnostic and therapeutic services, as ordered by the physician. Notify the customer’s attending physician and family when there is a change in the customer’s conditions. Obtain sputum, urine and other lab tests as ordered. Take and record TPR’s, blood pressures, accuchecks, etc., as necessary. Contact the customer’s physician for: Nursing assessment of change of condition. Medication orders or charges. To inform of incidents. To report abnormal results of Lab and X-ray. Anything else pertinent to the customers’ care and well-being. Ensure the nurses’ notes and monthly summaries reflect the care plan and is being followed. Attend staff meetings, etc., as directed. Perform other related duties as assigned.
