Qualifications Required: Graduate of an accredited school of professional nursing (or a program seeking accreditation). Current registered nurse licensure in the State of Ohio. Current AHA Basic Life Support for the healthcare provider certification or obtains within 90 days of hire. Must have: a. A thorough knowledge of nursing theory and practice, including basic knowledge relating to nursing such as biological, physical, psychosocial, and medical sciences and their applications. b. A thorough knowledge of the use and care of instruments, equipment and supplies related to nursing service or ambulatory medical practice. c. A thorough knowledge of the theory and principles of medical asepsis. d. The ability to maintain positive working relations with other employees and medical staff. e. A high degree of physical coordination and finger-hand dexterity. PRN and Float Staff must have a minimum of one year or number of years as determined by specific clinical unit, recent experience in area of clinical interest. All clinical nurses must possess or acquire within the defined period of time following placement into position, all unit-specific competencies as defined below: The American Heart Association (AHA) courses are the required certifications for: Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), Pediatric Advanced Emergency Assessment, Recognition and Stabilization (PEARS) The American Academy of Pediatrics (AAP) course is the required certification for Neonatal Resuscitation Program (NRP) Preferred: BSN and certification BMT: Step down experience Operating Room: One year of OR experience; Certified Nurse Operating Room (CNOR) certification Post Procedure Care Unit: One year PACU or critical care experience. Psychiatry: One year of prior psychiatric experience Radiology: Five years of critical care experience. Pediatric Advanced Life Support (PALS) certification. Cardio-Pulm Rehab: Two years prior experience in Cardiac-Pulmonary Nursing (outpatient or inpatient). Previous experience with the following: smoking cessation, wellness, exercise in special populations (i.e., diabetes, obesity, arthritis). TB Clinic Public Health or TB Experience. Excellent organizational and communication skills. Prior supervisory or project management experience. Physical Requirements: May sit, stand, stoop, bend, and walk intermittently during the day. May need to sit or stand for extended periods. See in the normal visual range with or without correction. Hear in the normal audio range with or without correction. Job Description Summary The Hospice RN assumes case management responsibility for an assigned caseload of hospice patients in their place of residence (e.g., home, nursing facility). This includes performing comprehensive assessments, developing and implementing the patient-centered plan of care, providing skilled nursing interventions for pain and symptom management, clinical documentation, and supervision of the Hospice LPNs and Aides. Essential Job Functions/Responsibilities Patient Care 1. Completes an initial, comprehensive and ongoing assessments of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Uses health assessment data to determine nursing diagnosis. 5. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 6. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 7. Counsels the patient and family in meeting nursing and related needs. 8. Provides health care instructions to the patient as appropriate per assessment and plan. 9. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. 10. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Communication 1. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 3. Communicates with community health related persons to coordinate the care plan. 4. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 5. Provides and maintains a safe environment for the patient. 6. Assists the patient and family/caregiver and other team members in providing continuity of care. 7. Works in cooperation with the family/caregiver and hospice Interdisciplinary Group Members to meet the emotional needs of the patient and family/caregiver. 8. Attends interdisciplinary group meetings. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. Position Qualifications 1. Registered nurse with current licensure to practice professional nursing in the state. 2. Graduate of National League for Nursing accredited school of nursing. 3. Maintains a current CPR certification. 4. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order. 5. Minimum of two years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 6. Self directed and able to work with minimal supervision. 7. Management experience not required. Responsible for supervising hospice aides. 8. Demonstrates excellent observation, problem solving, verbal and written communications; nursing skills per competency checklist. 9. Shows ability to organize and prioritize workload independently. 10. Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, and/or crouching. Visual acuity and hearing to perform required nursing skills.

Overview: As a Medical Assistant at Hometown Urgent Care, you will play a crucial role in supporting healthcare delivery by assisting healthcare professionals in various clinical and administrative tasks. Your contribution will be vital in ensuring the safety, welfare, and comfort of our patients. This position is full shifts (open to close) with focus on call off coverage and weekends. Duties: Responsible for taking patient history and obtain vital signs Perform basic testing or screenings such as: vision, audio, urine drug screens, breath alcohol, EKGs, pulmonary function testing. Perform necessary phlebotomy for collection of laboratory samples. Accurately complete corresponding paperwork/computer work for lab tests ordered on blood, urine, tissue, and culture samples. Able to perform subcutaneous, intradermal, and intramuscular injections and accurately complete paperwork according to policy for that procedure. Perform point of care testing such as: strep, mono, glucose, influenza, Urine pregnancy, Urinalysis, blood glucose levels. Dispense and document medications and other treatment modalities. Insure sterile technique, instrument cleaning and sterilization, proper use of the autoclave. Schedule and coordinate necessary records for ancillary care for patients. Assist providers with exams and testing (pelvic exams, eye irrigations and ear irrigations, I & D’s and splinting/ ortho-glass preparation Perform occupational medicine testing such as: urine drug screens, breath alcohol testing, audiogram, and DOT physicals Maintain confidentiality with all patient information (HIPAA). Ensures equipment is in working order. Reports equipment malfunctions to manager. Logs laboratory procedures completed. Processes related paperwork using computer equipment as directed. Responsible for patient call backs, scheduling physician referrals; specialist’s referrals and pharmacy issues; and patient’s education. Maintains examination rooms, sterilization of equipment and stocks necessary medical supplies. Performs duties to ensure good patient flow. Registration duties: explain to patients' accepted insurances and those out of network and knowledgeable of all pricing. Properly collect and record payments, complete registration screen in a timely manner. Travel to care centers within designated region Protects patients' rights by maintaining confidentiality of personal information Cleans and stocks the site with other members of the team Training other team members. Additional job responsibilities may be assigned. 1-2 years previous experience working in a physician practice or clinic setting is preferred.
