Confirms with family the discharge disposition, date and time for those patients who request it as well as those patients who do not have capacity. 1,2,5 Coordinates patient care with all hospital departments, community agencies and extended care facilities; acts as a liaison with other organizations to assess appropriateness of transfers. Ensures appropriate clinical documents necessary for post-acute placement are included within the referral. 2,3,4 Provides verbal and written explanation of patient’s plan of care, discharge plan, and post-discharge plan of care, encouraging patient and family to participate in plan development; assesses patient and family understanding of all plans . Provides patient/families choice for post-acute providers when indicated. Documents ongoing discharge planning process and discussions in patient medical record. Reviews with the SWA to ensure that discharge planning page in CarePort, is current and accurate for reporting purposes. In collaboration with the SWA, ensures that discharge planning documentation is current and up to date. As the clinical lead, should discuss with SWA when they need support to complete the activities of the day. 1,3,4 Performs monthly chart reviews. Monitors length of stay, timely scheduling of tests/consultations and reports barriers to department leadership for intervention. Job Description The AMI Navigator is responsible for coordinating care for patients diagnosed with acute myocardial infarction (AMI), with a primary focus on NSTEMI and medically managed AMI populations. This role ensures adherence to evidence-based treatment, focusses on LOS reduction strategies as well as readmission prevention strategies, enhances patient outcomes, and supports quality and regulatory requirements. The AMI Navigator facilitates seamless transitions across the continuum of care, reduces readmissions, improves compliance with guideline-directed therapy, and enhances the patient experience through education and coordinated follow-up. AMI Care Coordination Identify and track all AMI (primarily NSTEMI) patients from admission through discharge and post-acute follow-up. Coordinate diagnostic testing, cardiology consultations and post discharge physician visits, and treatment plans. Facilitate timely transitions between the ED, inpatient units, and outpatient care. Ensure appropriate level of care and escalation when clinically indicated. Clinical Oversight & Patient Management Conduct assessment of clinical, psychosocial, and discharge needs for AMI patients. Monitor adherence to guideline-directed medical therapy (GDMT), including: Antiplatelet therapy Statins Beta-blockers ACE inhibitors/ARBs (as appropriate) Identify high-risk patients for complications or readmission and ensure proactive intervention.
