In this role, the successful Nurse Practitioner uses clinical expertise to plan the delivery of safe, therapeutic, population-specific patient care through assessment, planning, implementation and evaluation. Works collaboratively within a multidisciplinary health team to promote wellness and optimize patient outcomes. Job Responsibilities: Performs other duties as assigned. Plans and organizes care to meet individual patient needs and to ensure appropriate clinical resource utilization per protocols, pathways, and other means. Orders customary laboratory, radiological and diagnostic studies per practice protocols, differentiates between normal/abnormal findings and follows protocols for abnormal findings, which may include referral to consulting physicians per customary pattern of the attending MD or with collaborating physician. Formulates the plan of care, along with the attending physician, patient and family, based on expected goals of care and length of stay. Works collaboratively with the attending physician, consulting physician(s), and other disciplines to identify, develop, and implement an appropriate plan of care that maximizes individual patient/ family preference and enhances quality, access, and cost-effective outcomes. Prescribes in-patient and discharge medications according to New York State law, including controlled substances with DEA authorization. Prescribes medical equipment, devices, physical and occupational therapy, and home health services per practice protocol. Practices as member of the allied health staff according to the rules and regulations of the medical staff and bylaws as outlined in the delineation of privileges. Evaluates treatment and health care plans for effectiveness and modifies per clinical standards and practice protocols. Considers needs and behaviors of specific patient age and cultural groups in all patient care. Promotes own professional growth and development in clinical and managerial role. Maintains current expertise in area of practice. Serves as a clinical expert and resource for the education of peers and other health professionals. Participates in the development and monitoring of patient outcomes per established practice protocols for purposes of quality and performance improvement. Works collaboratively with nursing and other disciplines in the development and implementation of clinical studies within the area of expertise. For Nurse-Family Partnership (NFP) only: Provides health education, social support and community linkages to meet the needs of pregnant and postpartum, first-time mothers and their infants. Also responsible for adhering to NFP national guidelines. • Assesses the physical, social and psychological needs and environmental conditions of the patient/client through professional knowledge, skills of observation, interviewing, and physical examination to develop a plan of care. • Understands, interprets, and implements reimbursement protocols; determines the billing source for patient/client care. Consults with management on complex/unique reimbursement cases. • Acts as the coordinator of care for the multidisciplinary team by providing and managing the care needs of patients/clients which includes making/confirming referrals to other disciplines/community agencies and ordering supplies/services and ensuring they are delivered as needed. • Documents content and process of all services provided in accordance with government regulations/requirements, and VNS Health policies, practices, and procedures, as well as NFP requirements as appropriate. Ensures that services follow the plan of care by ongoing assessment, planning, coordination, counseling, teaching and supervision of the delivery of care to meet the needs of the patient. • Oversees/supervises the care provided to the patient by Licensed Practical Nurses and Paraprofessionals through regular home visits in compliance with regulatory and VNS Health requirements. Assesses the patient’s condition, monitors the effect of the care plans on the patient/family health status, informs physician and other team members of changes in the client's needs and updates the plan of treatment, as needed. • Manages, plans, coordinates and executes the patient’s safe discharge from service. Ensures that physician’s orders, discharge summary and communication to team members, patient and family members adhere to VNS Health and regulatory requirements. • Communicates and consults with patient’s MD and follows-up on problematic situations affecting care such as abnormal clinical findings, medication discrepancies, or other factors that impact patient’s ability to comply with the plan of care. Manages and coordinates all aspects of assigned caseload. • Assists patients with activities of daily living, which may require positioning, moving, transferring and lifting patients of varying weights and physical conditions, with and without assistance and as appropriate, from family members and/or paraprofessionals. • For NFP Only • Establishes, implements, and revises a plan of care with input from the client and from appropriate multidisciplinary health team members (i.e., social worker, physician) based on goals mutually acceptable to the client. Maintain accurate medical records and health documentation. Communicate important health information to staff, families, and members of the interdisciplinary team. Train staff in OSHA standards, First Aid, medication support, and health and safety procedures. Perform nursing assessments, evaluations, and therapeutic interventions in accordance with professional nursing standards and applicable regulations. Participate in interdisciplinary team meetings and annual reviews. Supervise Licensed Practical Nurses (LPNs), AMAP staff, and other direct care personnel as appropriate. Promote a safe, healthy, and person-centered environment for all individuals served. Provide nursing oversight for multiple Day Program locations as needed. Perform other duties as assigned. Qualifications Current New York State Registered Nurse (RN) license required. Graduate of an accredited nursing program. Current CPR certification required (or willingness to obtain certification). Experience working with individuals with intellectual and developmental disabilities is preferred. Strong communication, organizational, and leadership skills.
