I. Qualifications Skilled Nurse from an Accredited program currently licensed and in good standing with the Pennsylvania State Board of Nursing. One to 2 years’ experience in nursing in an acute care setting or 1-year experience in home health, hospice or comparable experience is preferred. Desire to learn and demonstration of aptitude for the hospice philosophy and comfort care required. II. Responsibilities It is the responsibility of the Visit Nurse to coordinate and provide the highest level of hospice care to patients and families. The Visit Nurse works in conjunction with the Nurse Case Manager, Director of Nursing (DON) and the Interdisciplinary Team to: · provide hands-on clinical nursing, interacting with patients, family, physicians, Interdisciplinary Team members, and other health care professionals involved in the patient’s care. · assess and coordinate a pain/symptom management program formulated with the participation of family and patient, attending physician and the Interdisciplinary Team, utilizing pharmacological and non-pharmacological methods of pain/symptom control to enhance quality of life. · assist in facilitating freedom of choice, relief of suffering and death with dignity for patients facing end of life issues. · provide support and education for families based on the hospice philosophy of care. III. Orientation and TRAining Tender Care Hospice will provide orientation and training, as necessary, to assist the Case Manager Nurse in performing the responsibilities listed above. Training may be in-house or outside seminars. Evidence of orientation and training will become part of the personnel file. IV. On Call The Case Manager Nurse will be on call according to scheduling. During the hours when on call, the Visit Nurse is expected to follow On Call Protocol and to respond to all pages within 20 minutes. V. Accountability Reports directly to the Nurse Case Manager and Director of Nursing. The Nurse Case Manager will work under the direction of the Director of Nursing.

Other duties as required Overview The Children’s Regional Hospital at Cooper University Hospital is a pediatric facility that consists of several units that meet the diverse needs of pediatric patients: Pediatric unit, Pediatric Intermediate unit (PIMU), Pediatric Kostic Infusion Room (PKIR), and Pediatric Intensive Care unit (PICU). We provide care for pediatric patients aged 0 to 18 years. We offer subspecialty services through consultation and referral in areas such as Endocrinology, Genetics, Infectious Diseases, Neurology, Neurosurgery, Gastroenterology, Otolaryngology, Pediatric Surgery, Trauma, and Hematology. The Family-centered rounds are conducted daily with an interdisciplinary care team including physicians, professional nursing services, child life specialists, social services, and nutrition support. All our pediatric units are secured with restricted access for staff, with visitors gaining entry via monitored access. All pediatric patients wear security tags to assure their safety. Our facilities include a teen room, a spacious playroom for younger patients, and a family waiting room. Our Pediatric unit and PIMU have 38-bed capacity. The Pediatric unit is in Kellemen building on South 7th and offers 26 beds. Experience Required 2 years minimum RN pediatrics experience is required. Education Requirements Associates Graduate of NLN accredited School of Nursing. License/Certification Requirements Current NJ-RN License (Registered Nurse). Current NJ-BLS Certification (Basic Life Support). Current NJ-CNOR (Certified Nurse, Operating Room) preferred. Certain specialties may require advanced certification (i.e. ACLS, NALS, PALS). Special Requirements Baylor must work every weekend (defined as two (2) 12 hour shifts from 7pm Friday to 7am Monday) as determined by patient care and unit staffing needs. Baylor nurses will work either Saturday/Sunday days, Friday/Saturday nights or Saturday/Sunday nights. Baylor nurses work twenty-four (24) hours every weekend and are paid for thirty six hours (24/36).
