Responsibilities Patient Care 1. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. 2. This position would be responsible for providing nursing care to our HomeCare clients in Essex, Union, Hudson, Passaic and Morris counties in New Jersey. Our focus is on delivering compassionate and attentive care to individuals, in their private residences.
