Overview To support the Mission of Helio Health, Inc., the CTI Licensed Practical Nurse will provide comprehensive care management and coordination of services to Critical Time Intervention (CTI) program enrollees. The CTI model is a time-limited form of case management that focuses on helping people with severe mental illness (SMI) and a history of homelessness during a “critical time” transitional period in their lives. CTI Licensed Practical Nurse: CTI is designed specifically to enhance the continuity of care during transition from hospital to community care. Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, the Critical Time Intervention Licensed Practical Nurse is a care manager, but in addition to Care Management tasks, they resolve medical and rehabilitative needs, when within scope of practice, that impact the individual’s ability to meet recovery goals. The LPN will work on a Full-Time basis and using a hybrid work model, working in the community throughout Oneida County . The schedule is Monday-Friday, 8:00am-4:30pm, with one late shift per week (12pm-8:30pm), and some evening on call responsibilities. Pay Rate: $25.00 - $26.92 per hour In addition to Helio Health’s comprehensive benefits package, all Full-Time LPNs are eligible for a Sign-On incentive up to $4,000 ! Execute and review public health education and screening protocols with regards to tuberculosis, sexually transmitted diseases, hepatitis, and HIV prevention and harm reduction in accordance with Article 27-F of the Public Health law and under the Mental Hygiene law in coordination with the Medical Director/Nurse Manager. Implement and carry out a medically based group for patients on the above topics. Serves on special review and quality assurance committees as assigned. RESPONSIBILITIES: Registered nurses are the heartbeat of healthcare, wearing many hats every day to provide exceptional care to their patients. Their primary mission is to ensure that every resident receives the precise, compassionate care they need to thrive. Educating patients and family members on treatment and care plans, as well as answering their questions Confirms and transcribes all new medical orders on assigned unit prior to end of shift. Demonstrates adequate supervision of certified nursing assistants. Ensures POC documentation and quality resident care is provided on assigned residents. Functions as a leader for the CNAs; delegates in a manner that contributes to the excellence of the nursing unit to which LPN is assigned.
