Description Kindred Hospital Baldwin Park helps patients return to the lives they love. At our 91-bed hospital, our physician-led care teams provide critically ill patients with the specialized acute care and rehabilitation they need, for the time they need it - empowering them to take the next step in their recovery journey. We also offer an eight-bed ICU and three negative pressure rooms for treating particularly serious conditions. We are located near the I-605 and I-10 intersection in Baldwin Park with a number of shops and restaurants nearby for added convenience. Job Summary Assists professional nursing personnel in providing patient care in assigned area. Assists patients with activities of daily living, provides for personal care, emotional support and performs more complex clinical skills under the direction of professional nursing personnel. Essential Functions Consults with and keeps supervisor informed of unit activities, needs, and problems related to patient care. Provides general nursing care such as positioning the patient, lifting, turning, applying/utilizing special equipment, assisting in the use of bed pan, urinal or commode, ambulating the patient, pre operative and post operative care, and application of binder or anti embolic stockings, bed bath, oral/hair/back care, changing bed linens, cleaning over bed table, and bedside stand, straightening patient room and other general care necessary during the shift. Takes and records temperature, pulse, respiration, weight, height, blood pressure, and intake and output measurements accurately. Documents in a timely manner. Prepares patient room and bed for admission and transfers and ensures all necessary equipment is in room. Maintains an attractive and comfortable environment for patients with special consideration to cleanliness of room, ventilation, and lighting. Assists in patient admission, transfer, and discharge procedures. Assist with handling and care of patient belongings and other personal property. Answers patient call lights, telephone, and paging system and responds as appropriate. Under the direction of the Registered Nurse, performs routine treatment procedures such as enemas, application of appliances for heat and cold. Able to release, remove, and reapply restraints to patient under the direction of an RN. Ensures patient is in proper position and alignment. Assist Registered Nurse and/or LVN/LPN with complicated patient treatment procedures. Serves and removes patient meal tray in timely manner. Assists with feeding and/or preparing items such as opening milk container, cutting food for patient. Knowledge/Skills/Abilities/Expectations Must read, write and speak fluent English. Must have good and regular attendance. Excellent oral and written communication and interpersonal skills Basic computer knowledge. Able to organize tasks, develop action plans, set priorities and function under stressful situations. Overview JOB SUMMARY: Responsible for the delivery of patient care through the nursing process of assessment, diagnosing, planning, implementation and evaluation, provided to the women and newborns during the antepartum, intrapartum, postpartum and neonatal stages of the childbirth process. Assist with bedside procedures, enforcing mandatory "time-out" safety protocols. Care Coordination & Handoffs: Conduct detailed bedside change-of-shift reports to ensure continuity of care, and participate in purposeful patient rounding. Job Summary: Works collaboratively with an assigned panel of physicians to manage the patients specialized needs. The managing team does differ according to the chronic disease. Duties include assessment to identify member needs and development of specific care management plan to address needs. In conjunction with the Physician, implements care/treatment plan by coordinating access to health services across multiple providers/ disciplines, monitors care, makes determination to arrange transportation and transfer patient if indicated, identifies cost-effective measures, makes recommendations for alternative levels of care and utilization of resources, promotes self-care management and ensures paper work is completed. Is an indirect caregiver. Complies with other duties as described. Must be able to work collaboratively with the Multidisciplinary team. Essential Responsibilities: Evaluates and identifies members needs. Interfaces with Primary Care Physicians, Specialists and various disciplines on the development of case management plans/programs. Monitors and evaluates the effectiveness of the case management plans and modifies as necessary. Coordinates the interdisciplinary approach to providing continuity of care, including utilization management, transfer coordination, discharge planning and obtaining all authorizations/approvals/transfers as needed for outside services for patients/families. Acts as a clinical liaison, per their specialty, with outside agencies such as County CCS, non-plan facilities, outside providers, employers and/or workers compensation carriers and third party administrators. Prepares reports, communicates program changes to appropriate staff and develops protocols in accordance with state regulations. Acts as a patient advocate and educator to assure that the patient has the knowledge to care for his/her condition and patient is educated and empowered to be responsible for participating in the plan of care. Develops individualized patient/family education plan focused on self-management, delivers patient/family education specific to a disease state. Develops and updates training and educational materials and presents to appropriate staff, members and families. Facilitates patients return to normal daily activities by teaching and making appropriate referrals for outside services/continued care. Consults with internal and external physicians, health care providers, discharge planners, and outside agencies regarding continued care/treatment or hospitalization or referral to support services or placement. May need to facilitate transportation and housing arrangements for patient. Coordinates transmission of clinical and benefit treatment to patients, families and outside agencies. Participates in data collection and analysis of clinical outcomes of care and customer satisfaction standards. Participates in the formulation and implementation/monitoring of action strategies and outcomes of care or customer service. Ensures that accurate records are maintained of the care associated with each patient. Interprets regulations, health plan benefits, policies, and procedures for members, physicians, medical office staff, and contract providers and outside agencies.
