JOB DESCRIPTION: The Certified Nursing Assistant works directly under the supervision of the Registered Nurse or Licensed Practical Nurse (LPN) and performs duties in a relation to the personal needs of the resident, care of supplies and equipment, and maintaining the cleanliness, comfort and safety of the resident in his/her environment. Engages in cluster feeding while seated. Monitors daily intake and keeps nurses informed. Records food intake on Nutrition Work Sheet. Making Beds Uses recommended amount of linen and diapers. Disposes of linen in proper manner. Properly utilizes clean and dirty linen carts. Secures call bells to bed. Beds are made in proper manner. Knowledgeable in procedure for linen precaution. Safety of Residents: Make sure, if restraint is used, that it was indicated on assignment and the proper type is applied. Restraints are checked at the beginning and end of each shift. Supervises ambulation of resident when needed. Proper attention is given that side rails are up on bed when needed. Completes dining room duty as posted by charge nurse. Is alert for spills on floor. Answers call bells as soon as possible. Makes rounds frequently. Reports need for repair and safety matters to charge nurse. To Ascertain Vital Signs Follows procedure in taking TPR correctly. Adheres to the proper procedure for cleaning thermometers/IVAC. Rehabilitation Assists in ambulation, transfers, ROM exercises and good body alignment where indicated. Uses adaptive devices properly. Cooperates with all rehab disciplines. Keeps accountability sheets up to date with signatures as required. Testing of Urine and Collecting of Specimens Has knowledge of proper procedure. Records results. Informs nurse of results. Assignments Receives daily report from nurse. Consults assignment for special duties and sets proper priorities for following through on assignment. Is familiar with and takes proper action in meeting physical needs of resident. Routinely checks and keeps in order all residents' belongings: i.e. clothing, glasses, hearing aid, prosthesis aid, adaptive devices, etc. Ensures that residents on turning schedule is positioned every two hours or as ordered, and documented. Ensures that residents on toileting schedule are assisted as needed, and documented. Resident is kept clean and dry. Gives assistance to co-workers when needed. Uses good handwashing techniques. Personal Appearance Meets uniform code. Wears official facility staff ID tag at all times when on duty. Excessive jewelry not to be worn. Cooperation Works well with co-workers and supervisors. Maintains appropriate relationship with residents, families and peers. Gives assistance to other departments: i.e. recreation, when necessary. Communicates well with other staff members. GENERAL JOB SUMMARY An exempt clinical position where the nurse practitioner (NP) is responsible for providing direct patient care. The NP participates as a leader of the skilled nursing facility (SNF) care team. Visits managed care and fee-for-service patients at skilled and long-term levels of care in designated SNFs facilities. Provides appropriate evidence-based geriatric medicine. Coordinates care with hospitalists, primary care physicians and care managers. Makes home visits as directed by the medical staff to meet patient needs and provide continuity of care. ESSENTIAL JOB FUNCTIONS Maintains privileges in multiple Nursing Homes as directed by ACA Maintains license and malpractice insurance Consults supervising attending as needed Documents patient visits electronically at least 90% of the time Participates in documentation and other quality improvement programs Available via phone weekdays 8am- 7pm and when on call. Will reviews, approves, and modifies admission orders Creates a detailed admit note for each admission within 24 hours of patient admission to SNF, including medication reconciliation Initiates/documents Advanced Directives Determines if Health Care Proxy status is correct and invoke if appropriate On weekends, takes call for admissions and see new patients within 24 hours of admission on a rotating basis with other practitioners in the program.
