Description As an integral part of the clinical care team, the Patient Care Associate is responsible for patient care and support activities promoting patient focused care within the Specialty Service. The Patient Care Associate supports the registered nurse in patient care activities, such as: assisting patient with activities of daily living, collecting specimens and performing procedures; maintaining a safe, clean environment; and other duties as needed. Responsibilities A. Clinical/Technical/Service • Demonstrates the ability to perform clinical/technical/service/administrative tasks • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. • Performs and records patient’s vital signs, such as blood pressure, pulse, temperature and respirations. Documents intake and output and other data as assigned • Obtains patient’s rating on pain scales as a fifth vital sign in all areas except Pediatrics • Performs clinical procedures, such as specimen collection including blood glucose, venipuncture, EKG, application of compression devices, post-mortem care, and maintenance of skin integrity. • Sets up for selected procedures and assists RN’s accordingly. • Assists patient with activities of daily living, such as feeding, bathing and/or toileting. Position Summary We are seeking a compassionate and experienced Admissions Registered Nurse (RN) to join our hospice team. The Admissions RN is responsible for evaluating patients for hospice eligibility, completing admission assessments, coordinating the start of care, and educating patients and families about hospice services. This role requires excellent clinical judgment, strong communication skills, and the ability to work collaboratively with interdisciplinary team members to ensure a smooth transition into hospice care. This position will oversee the Bergen/Essex/Union county area. The hours are Monday-Friday 11am-7pm Responsibilities Conduct comprehensive hospice admission assessments in patients' homes, facilities, or hospitals. Determine hospice eligibility in collaboration with the hospice physician and interdisciplinary team. Complete all required admission documentation accurately and in a timely manner. Develop and initiate individualized plans of care. Educate patients and families on hospice philosophy, services, medications, and expectations. Coordinate admissions with referral sources, physicians, hospitals, nursing facilities, and case managers. Obtain informed consents and ensure all regulatory requirements are met. Collaborate with clinical staff to facilitate seamless transitions of care. Maintain compliance with Medicare, state, and agency regulations. Participate in on-call or weekend rotation as assigned (if applicable). Provide compassionate emotional support to patients and families during the admission process. The Nurse Practitioner provides clinical care for participants within the PROS program while maintaining a strength-based recovery-oriented perspective. The Nurse Practitioner works collaboratively with the psychiatrist to provide patient assessment and diagnosis, formulate treatment plans, prescribe medications, order laboratory tests, interpret test results, provide patient education, prepare medical records and refer patients for inpatient care in accordance with statutes, regulations and protocols regulating the profession.
