Description Northwestern Medicine has an exciting opportunity for either a motivated Acute Care Nurse Practitioner or Physician Assistant to work with our Palliative Medicine team. This position provides symptom management for patients with serious or life limiting illnesses and leads goals of care conversations with patients and families. This position is 100% inpatient. Schedule: Monday-Friday No nights required. Participation in a weekend on call rotation every 6 to 8 weeks and a shared holiday rotation with the team. Essential Functions: Demonstrates clinical expertise and expanded knowledge through patient care activities which may include diagnosis and management of a wide range of health problems. Responsible for health outcome of an assigned patient population. Confers with collaborating physician and other health professionals in area of clinical expertise. Develops appropriate educational systems for patients, families and staff in areas of expertise. Mentors graduate students in aspects of advanced practice and patient care. Actively participates in quality improvement and/or research pertinent to area of expertise. Responsibilities : Conducts comprehensive appraisals of patients' health status by taking health histories and performing physical examinations in clinical area. Provides direct clinical care to patients. Prescribes/orders medications, therapies; orders/interprets laboratory tests, x-rays and procedures according to clinical privileges. Evaluates patients to determine their need for further health care. Provides comprehensive teaching to patients and families. Documents in the medical record all care and education provided and/or procedures performed for which the Licensed Advanced Practice Provider is responsible. Refers to other disciplines as appropriate. Consultant to other disciplines in areas of expertise. Integrates research based knowledge and clinical expertise in decision making. Report all concerns, complaints made by the customer or their family to the Administrator. Counsel/discipline nursing personnel as requested or as necessary. Make daily rounds to ensure nursing personnel are performing required duties, and to ensure appropriate procedures are being followed. Make physical rounds to all customers daily. Make rounds with physicians as necessary. Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the department. Attend and participate in continuing education programs. Recommend to the Director of Nursing the equipment and supply needs of the department. Place orders for medications and treatments as necessary following established budgetary guidelines. Utilize PCC 24-hour summary concerning recommendations/concerns related to customer care needs. Fill out complete accident/incident reports. Encourage attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies. Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the customer, as well as the customer’s response to the care. Complete required assessments upon the customer’s admission, transfer, and/or discharge. Receive telephone orders from physicians and record on the Physicians’ Order Form and P.O.S. Record new/changed diet orders. Forward information to the Dietary Department. Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, care of the dead/dying, etc., as required. Participate in the orientation of new customers/family members to the facility. Arrange for diagnostic and therapeutic services, as ordered by the physician. Notify the customer’s attending physician and family when there is a change in the customer’s conditions. Obtain sputum, urine and other lab tests as ordered. Take and record TPR’s, blood pressures, accuchecks, etc., as necessary. Contact the customer’s physician for: Nursing assessment of change of condition. Medication orders or charges. To inform of incidents. To report abnormal results of Lab and X-ray. Anything else pertinent to the customers’ care and well-being. Ensure the nurses’ notes and monthly summaries reflect the care plan and is being followed. Attend staff meetings, etc., as directed. Perform other related duties as assigned.
