Overview Lancaster Nursing & Rehabilitation Center is excited to welcome an experienced and compassionate LPN Wound Care Coordinator to our nursing team! This is a full-time, hourly clinical position focused on coordinating and supporting the facility’s wound care program. The Wound Care Coordinator works closely with the Director of Nursing, nursing staff, physicians, and the interdisciplinary team to promote skin integrity, support wound healing, and help ensure high-quality resident care. The typical schedule for this position is 8:00 AM–4:00 PM, with hours subject to change based on facility needs and the direction of the Director of Nursing. Key Responsibilities Coordinate and support the facility’s day-to-day wound care program in accordance with nursing standards, facility policies, and applicable regulations. Complete accurate and timely wound documentation, including nursing notes and required wound tracking. Perform and document skin and body assessments on new admissions as required by facility policy. Assess residents at increased risk for skin breakdown and help implement appropriate pressure-reduction and prevention interventions. Qualifications Graduate of an accredited school of nursing. Current, active, and unencumbered Licensed Practical Nurse (LPN) license in Pennsylvania . Previous long-term care experience preferred. Wound care experience preferred. Wound care certification or specialized wound care training is a plus. Knowledge of nursing practices, procedures, and regulations governing long-term care.
