SUMMARY: The primary purpose of this job position is to assist in the planning, implementation, and evaluation of recreational, social, intellectual, emotional and spiritual programs, in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. Observe resident attendance, mood, behavior, and degree of involvement so that the facility activities & resident progress are evaluated, by noting, reporting and charting resident behavior. Assume the authority, responsibility & accountability of Activity Assistant/Aide. Ensure that all charted activity progress notes are informative and descriptive of the services provided and indicate the resident’s response to the service. Develop & maintain a good working rapport with other departments within the facility to assure that daily activities can be performed without interruption. Create & maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the facility. JOB SUMMARY The Memory Care Aide assists the Memory Care Assistant Director in providing a holistic, therapeutic, person-centered Memory Care Program on the Memory Care Unit. Assess, create, implement, and maintain any communication needs of residents, such as language services, communication boards, assistive devices, etc. Educate residents, family and staff utilizing these mechanisms to provide ample communication. Become knowledgeable of each individual resident’s background, life history, disease, medical and behavioral care needs, and preferences in detail to develop and ensure an appropriate person-centered plan of care. Assist in providing a therapeutic, person-centered memory care activity program that constantly strives to enhance the quality of life of each resident, meets the Memory Care Program Mission and Philosophy, and upholds the Memory Care policies and procedures and all state and federal standards. Provide both group and 1:1 activity programming. Complete all required activity participation documentation. Encourage family involvement through volunteering and visitation. Participate in monthly Family Council Meetings as appropriate. Follow-up timely on any concerns brought forth. Provide a clean, safe, non-hazardous unit for the residents and staff. Assist in providing an individualized symptom management tracking system as needed per resident, including development and maintenance of individualized, non-pharmacological interventions implemented. Assist in providing psycho-social programming in conjunction with the Memory Care group and 1:1 programming, as assessed as appropriate for residents. Assist with involvement in Accidents and Incidents process, in conjunction with the inter-disciplinary team to determine root cause, develop processes, and/or develop care plans that will decrease accident/incident occurrences. Complete all required documentation. Maintain all program policies and procedures and state and federal regulations. Provide feedback to the Memory Care Director for monitoring achievement of the facility and Memory Care Program goals through a team approach. Be flexible to meet the change in residents’ needs. We need to adapt to their needs rather than expecting them to adapt to our needs. Provide emotional, caring support to residents, families, and the inter-disciplinary team. The RN Case Manager is responsible for traveling to a patient’s home to administer the services dictated by their physician and At Home Quality Care while helping patients maintain their independence and quality of life. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES: Patient Care: 1. Assess and chart observations of the patient's condition and needs at each home visit. 2. Complete evaluation tasks, including medication reviews and vital signs. 3. Initiate the Plan of Care and makes necessary revisions as patient status and needs change. 4. Provide education to patients and family members on proper home health procedures and strategies. 5. Develop a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive and rehabilitative nursing actions. 6. Administer medications and treatments as prescribed by the physician. 7. Provides health care instructions to the patient as appropriate per assessment and plan of care. 8. Identifies discharge planning needs as part of the care plan. Prepares patient and family for discharge. 9. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. 10. Coordinate with physicians, physical therapists, occupational therapists, MSWs and other individuals in the patient care plan. Perform a supervisory visit with CNA (when part of the care plan) once every two (2) weeks. Communication: 1. Prepares clinical notes and updates the primary physician when necessary. 2. Communicate with the physician and office regarding the patient's needs and report any changes in the patient's condition; obtain/receives physician's orders as required. 3. Communicate with community health-related persons to coordinate the care plan. 4. Reports with home office on any updates, concerns and issues regarding the patient. POSITION QUALIFICATIONS: 1. Graduate of an accredited school of nursing. 2. Current licensure in state of Illinois and current CPR certification. 3. Management experience not required. Responsible for supervising home health aides. 4. Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, nursing skills per competency checklist. 5. Prolonged or considerable walking or standing. Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills. 6. Must be a licensed driver with an automobile insured under the state of IL. Automobile is in good working order. 7.
