Assistant Director of Nursing RN Skills and Abilities: Proven strong management skills, including delegating responsibility to subordinates, performance management, conflict management, and leading through others. Must have good financial and management skills. Must be able to adapt to a flexible work schedule and occasional overnight travel. Ability to read, write, speak, and understand English. Meets all health requirements, as required by law. As a Weekend In-House Nurse Practitioner , you will be supporting the primary provider of internal medicine services within a Skilled Nursing Facility (SNF). Master's Degree in Nursing from an accredited program. Strong clinical background in primary care and preventive medicine. Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings. Ability to engage and communicate effectively with elderly and chronically ill patients and their families. Ability to maximize onsite time to develop robust caseloads Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus. Key Responsibilities: As the Weekend In-House Nurse Practitioner , you will provide essential care and leadership within the facility, including: Ensuring continuity of care by communicating with the primary In-House Nurse Practitioner Conducting new admission evaluations and monthly stability visits for long-term residents Performing comprehensive physical examinations and evaluating changes in patient conditions Prescribing medications and treatments as needed Developing patient care policies and facilitating referrals to specialists Coordinating advanced care planning and discharge visits Ensuring timely communication and documentation of patient assessments, treatment plans, and follow-ups Supporting the physician-patient relationship and addressing gaps in care Our NPs work closely with the interdisciplinary team, taking a leadership role in patient care, collaborating with physicians, and ensuring high-quality service delivery in our facilities.
