Scope & Impact Under the supervision and medical direction of an anesthesiologist, you will perform comprehensive anesthesia care, including pre-operative evaluation, appropriate pre-op orders under approved procedures, administration of monitored, regional, and general anesthesia, intra-operative monitoring, safe emergence, and post-operative support. You will assess medical history, physical findings, laboratory tests, ECGs, chest X-rays, medication interactions, airway risks, and indications for delaying elective surgery or obtaining specialty consultation. You will determine appropriate monitoring needs, including arterial lines and Swan-Ganz pulmonary artery catheters, and safely place venous and arterial monitoring lines and devices as directed. You will also contribute to education, including supervision and progress assessment of student anesthetists when assigned. Strong command of anesthesia pharmacology, dosing, side effects, contraindications, and relevant cardiac, respiratory, neurologic, endocrine, and renal physiology. Skill in informed consent conversations, complete documentation, anesthesia records, patient monitoring, airway management, laryngoscopy, intubation, and safe patient transport/handoff. Follows established protocols for patients at high risk for falls and skin breakdown. Follows appropriate Code Blue notification procedures. Functions in a Code Blue quickly and calmly providing CPR, administering and documenting medications/interventions accurately. Effectively prioritizes clinical interventions and care. Communicates significant changes in patient’s condition/status in a timely manner. Understands actions, side-effects, contra-indications of drugs. Follows five “rights” of medication administration. Adheres to medication policies, practices and standards. Administers medication, intravenous therapy and fluid management only on physician request and guidance. Obtains necessary training prior to initial equipment use. Assures equipment is in operating order prior to use. Differentiates between patient complications and equipment malfunction. Uses the following equipment in accordance with manufacturer’s operating instructions, including but not limited to: IV Pump Glucoscan PCA Pump Tube Feeding Pump Chest Tube Suction Defibrillator Other duties as assigned. The nurse conducts clinical assessments, coordinates care, ensures medication safety, assists with closing quality gaps, and promotes member engagement through regular outreach and education. What You'll Do Comply with Astrana and department policies and procedures. Assess the medical, social, and behavioral needs of members. Complete comprehensive member assessments within required timeframes and identify individuals who may benefit from case management using clinical judgment, observed health or psychological changes, and assessment-driven triggers. Ensure timely, accurate, and complete documentation in all systems. Develop and implement an individualized case management plan in collaboration with the member, caregiver, social worker, physician and/or other involved healthcare professionals and the member’s support person to address member needs and goals. Collaborate with hospitalists, inpatient case managers, and discharge planners to establish safe discharge plans beginning at admission. Review medical records and/or other documentation to determine medical necessity. Conduct medication reconciliation post-discharge. Review medications for accuracy, missing medications, duplications, and potential interactions. Promote medication adherence through reminders, education, and barrier reduction. Close open quality measure gaps. Review member profiles and medical records to identify open care gaps and assist in closing the gaps when applicable. Attend and participate in company meetings, team meetings, and IDT meetings. Other duties and tasks as assigned. Qualifications Active Licensed Vocational Nurse (LVN) license in California with the ability to become licensed in other states, including but not limited to, NV, AZ, and HI. Proficiency with Microsoft Office applications and experience navigating electronic medical record (EMR), care management, or other healthcare technology platforms.
