Communicates with other health related persons to coordinate care plan. Participates in on-call duties as defined by the on-call policy. Ensures that arrangements for equipment and other necessary supplies and services are available. Instructs, supervises and evaluates home health aide/personal care aide care provided every 14 days or as stipulated by law and regulation. Instructs, supervises and evaluated LPN/LVN care provided at least every 30-days. Exercises professional management. And coordinates care between and among involved parties in home, community providers and pediatric tertiary specialists. QUALIFICATIONS: Bachelor's degree of Nursing from an accredited college/university Current active and unrestricted nursing license Pediatric care experience in the acute and/or home care environment Valid driver’s license Current certification in Basic Life Support for Health Care Professionals.
