Cleans and sterilizes instruments. Inventories and orders medical supplies and materials. Gives injections or treatments, and performs routine laboratory tests. Keys data into computer to maintain office and patient records. Assist doctors with charting Chart auditing, CPT and ICD-9 review Venipunctures, EKG's, Vital Signs, Blood draws, and Injections Prep patients for examination and treatment, and assist during minor office procedures Prepare exam and treatment rooms (instrument preparation and sterilization, prescription refills, faxing, and filing) Knowledge, Skills, Abilities and Other Qualifications Medical Assistant experience CPT I certification Experience performing EKGs, blood draws, vital signs and IM injections. JOB DESCRIPTION Job Summary Provides medical assistant support for Care Connections team. Responsible for contacting members following appointments/services rendered to ensure member understanding and navigation of the health care system, and coordination support for follow-up care. Also provides assistance for other administrative clinical support tasks. Contributes to overarching strategy to provide quality and cost-effective member care. Essential Job Duties • Coordinates care between members and providers after appointment/service delivery to support member navigation of the health care system, needs related to follow-up care and highest quality care/desired member health outcomes. • Communicates with Care Connections providers to clarify orders/requests and assists with escalation of issues to appropriate team(s) when unable to resolve independently. • Supports processing and follow-up for care management assignment and primary care provider referrals for high-risk members. • Escalates member issues related to prior authorizations for medications and durable medical equipment. • Communicates lab results and other exam findings with members and primary care providers. • Communicates with nurse practitioners on member health issues to ensure timely follow-up and response. • Supports care coordination and intervention by sending electronic messages/faxes to primary care providers. • Closes gaps in health care by preparing member lab result letters and coordinates follow-up on lab results. • Fulfills nurse practitioner supply orders and maintains adequate nursing supply inventory. • Performs administrative tasks such as telephone and email response, documentation follow-up and updates to member records. • Facilitates follow-up calls to members to ensure positive member experience/outcomes and ensure needs are met. • Assists with special projects under the direction of clinical leadership. Job Overview The LVN Case Manager will provide daily care coordination with concurrent review for patients in the hospital or Skilled Nursing Facilities (SNF) telephonically. This position will also provide case management to patients who are admitted to the hospital or SNF, and those patients who may need to be enrolled in ambulatory case management. The LVN case manager will be responsible for identifying (California Children Services) CCS cases, handling transfers, and retro reviews. Works as part of an interdisciplinary care team coordinating care and collaborating with social work, ambulatory case management, Regional Medical Directors, Hospitalists, the hospital nursing personnel, and the physician in the clinic. Minimum Requirements Graduation from an accredited LVN nursing program. Current valid License as a Licensed Vocational Nurse Experience in and willingness to be part of a multi-disciplinary team. Support prenatal and postpartum visits by obtaining relevant histories, vital signs, screening information, and other clinical data and assisting providers with routine prenatal care. Perform medication reconciliation, identify discrepancies or concerns, and communicate findings to the provider.
