CareOne at Essex Park Job Duties The Unit Manager is responsible for the day to day coordination oversight of all aspects of his/her assigned clinical unit. Job duties include, but are not limited to: Ensure that required staffing levels are met for each shift on a daily basis needed to render care for the number of residents assigned to her/his responsibility. Make sure that staffing level adjustments are made each shift as needed and the Federal required posted schedule is adjusted to reflect current actual staffing. Attend, participate and lead the weekly resident "At Risk" meeting ensures resident care and condition concerns are identified, interventions are appropriate to cause and follow up is completed. Attend and participate in morning clinical review meeting. Attend and participate in PPS meetings as indicated. Attend and participate in monthly QA and A meetings, as assigned, and assist with follow up for any identified problems. Monitor provision of resident care by licensed nurses and C.N.A.'s ensure required documentation is completed. Review 24- hour resident change in condition report daily ensure necessary follow-up occurs including appropriate interventions implemented, physician and family notified and required documentation has been completed. Review resident medical records and documentation to ensure clinical programs are in place per policy and procedures. Identify educational needs of staff discuss findings with the Facility Educator and work collaboratively to meet these needs. Participate on other committees as requested by the Administrator/Director of Nursing Services. Ensure that resident care and documentation is in compliance with state and federal rules and regulations. Comply with and ensure that staff is in compliance with HIPAA rules and regulations, reporting any known violations to the Administrator and Director of Nursing Services. Ensure work assignments are made for assigned staff, including breaks and lunches and staff is compliant with assignments. Initiate disciplinary action as necessary for staff members in collaboration with DON/ADON. Communicate with the appropriate department manager/DON and participate with follow up and resolution to any grievances/concerns from personnel, residents or family members. Ensure physician orders are being followed. Assist the DON/ADON with preparing performance evaluations as directed. Greet new admissions and families and assist nursing staff with admission process as needed. Within 24 to 48 hours, ensure that all admission documentation is complete including all required clinical assessments and care plans. Monitor medication passes and treatment schedules to ensure that medications are being administered as ordered and that treatments are provided as scheduled. Provide direct nursing care as necessary. Report problem areas to the Director of Nursing. Assist in developing and implementing corrective action. Attend and participate in continuing education programs to remain current with standards of nursing practice and to maintain licensure requirements. Ensure that your nursing staff follows policy and procedures, including appropriate dress code. Ensure that nursing staff participate in all fire safety and disaster preparedness drills. Ensure that nursing personnel are follow established infection control procedures when isolation precautions become necessary. Ensure that all personnel operate nursing equipment in a safe manner. Participate as needed and/or directed in collaboration with the Resident Assessment/Care Plan Coordinator in the development of a written care plan (preliminary, interim and comprehensive for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished and which professional service is responsible for each element of care. Ensure that all nursing care is provided in privacy and that nursing service personnel knock before entering the resident's room. Report and assist with investigation of any allegations of resident abuse including misappropriation of resident property. Assist Social Services with ensuring that an Advance Directive/Code Status has been obtained and documented in appropriate places per policy. Participate in the annual survey and/or complaint survey processes as directed by Administrator/DON and assist as directed with any plans of correction required. Position Requirements Bachelor's degree preferred and/or must be a graduate of an approved RN program. Must be an actively licensed RN in assigned state(s) of employment Previous supervisory experience. Two years of LTC experience. Acute Care experience preferred. Together with the medical provider, the RN Case Manager collaborates with all members of the care team, focusing on the delivery of efficient, high-quality care. This position ensures the appropriate utilization of clinical resources with a goal of a safe and timely discharge for the patient. This role navigates health system services to support effective transitions while advising the team on healthcare industry compliance. The assessment should also identify the needs of the patients, acknowledge current resources available, and anticipate future resources needed to facilitate successful transitions. Navigates the care delivery system while collaborating with the physician and other clinical departments by ensuring that tests, treatments, consults, and procedures are appropriately indicated and performed timely. Articulates the plan of care and communicates this plan to other care team members and patient/caregiver. Intervenes to maintain care progression when a deviation in the plan occurs. Influences positive outcomes by communicating the plan of care, expected discharge date, and transition needs to the patient/caregiver and team, thereby enhancing patient and staff satisfaction. Tasks: Creates and coordinates the overall transition plan of care based on initial assessment and concurrent collaboration with social workers, direct care providers, other hospital departments, external service organizations, agencies and healthcare facilities, community care and navigation services, and the patient and family/caregiver. Participates in daily multidisciplinary rounds incorporating evidence/best practice milestones in the plan and communicates that plan to the health care team. Apprises the interdisciplinary team of the estimated length of stay, care progression barriers, and anticipated disposition. Identifies what is needed from the team to facilitate the plan. Facilitates smooth care transitions by ensuring appropriate clinical follow-up is arranged and referrals to proper post-acute providers are initiated. Communicates the plan effectively with the patient and family/caregiver making certain that they have resources for success post-discharge. Understands organizational goals for the length of stay and unplanned readmissions. Tasks: Identifies appropriate clinical guidelines and directs the care plan to establish the anticipated length of stays and appropriate patient status. Proactively interfaces with the payer, where required, verifying coverage/benefits for anticipated discharge needs. Identifies patients that are at readmitted or at high risk for unplanned readmissions and initiates appropriate interventions. Identifies organizational resources within the community and engages those resources as necessary. Documents avoidable days (if not captured by another Care Transitions Team member), case management assessments, and care plans in a thorough and timely manner, per department policy. Ensures appropriate care provider documentation to support the patient’s anticipated discharge plan of care. Escalate deviations from the plan to the Physician Advisor as appropriate. Possesses effective verbal and written communication, relationship-building techniques, and negotiation skills. Tasks: Completes clear and concise documentation of the care plan and communicates this to the interdisciplinary team and the patient-caregiver. Identifies and communicates any problems or issues affecting patient flow, patient satisfaction, safety, length of stay management, or outcomes to the department director and/or appropriate key stakeholder. Functions as a resource for governmental and health care industry regulations and ensures compliance, communicates standards to the interdisciplinary team. Informs the patient and family/caregiver of the plan of care and the plan progression. Facilitates communication with the providers and encourages open dialogue. Maintains current knowledge of organizational policies, care transitions, and clinical trends, as well as regulatory requirements for clinical care, discharge planning, and authorization for post-acute services. Tasks: Attends and contributes to departmental staff meetings. Participates and contributes to multi-disciplinary committees and other committees or workgroups as directed. Manages quality indicators such as avoidable delays, length of stay, resource utilization, patient satisfaction, patient flow, outlier management, and readmissions while suggesting strategies to improve organizational/departmental performance. Applies the skill necessary to implement the medical/nursing plan of care, nursing interventions, medication administration, and procedures necessary for the care of the patient under the direction of the Registered Nurse. 2. Provides direct patient care within the primary nursing model to ensure continuity of care for all patients within their caseload in a safe efficient manner maintaining the patient's rights and dignity at all times . 3. Performs technical aspects of patient care (medications, treatments, etc.) safely, according to the established policies and procedures and Nurse Practice Act. 4. Assess and reassess pain . Utilizes appropriate pain management techniques . Documents pain management. 5. Communicates all changes in the patient's condition immediately to the Registered Nurse. 6. Bases all nursing interventions on pathophysiological and psychosocial rationale. Delivers care in a fiscally responsible manner. 7. Establishes a positive relationship with the patient and family. Introduces self to patients and families as the patient's nurse. Provides teaching and counseling for patients/families according to their needs and ability to understand. Documents teaching on the appropriate teaching tool . 8. Reviews/educates the discharged patient and family to ADL's, medications, treatments, and provides appropriate discharge instruction sheet(s). 9. Documents all nursing interventions and the patient's response in the medical record for all assigned patients. 10. Delegates responsibilities to others with an understanding of their roles, knowledge and capabilities. 11. Participates in performance improvement activities as requested , utilizing performance improvement principles to support and improve patient care . 12. Holds self accountable for professional practice and presents self in a calm and professional manner.
