Responsibilities The Registered Nurse is responsible for the delivery of safe patient care utilizing the nursing process of assessment, planning, implementation and evaluation. Provides direct patient care within the scope of practice. The Registered Nurse also directs and guides patient/family teaching and activities of other nursing personnel while maintaining standards of care in Telemetry Unit. The Registered Nurse is directly responsible and accountable for the care given to their assigned patients. They communicate with the physician about changes in the patient’s clinical condition including cardiac monitoring, results of diagnostic studies and symptomatology. Is able to respond quickly and accurately to changes in condition and/or response to treatment. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. Current and valid state RN License. 2. Current BLS certificate upon hire and maintain current. 3. Basic Arrythmia Interpretation within 30 days of hire. 4. Current ACLS certificate 30 days upon hire and maintain current. 5. Previous Acute Care hospital experience preferred. 6. Ventilator Management experience preferred. 7. Medical-Surgical Nurse-Board Certified (RN-BC) or similar certification; preferred. 8. Bachelor of Science in Nursing (BSN) preferred. May oversee the accurate collection and entry of data specific to patient population. May prepare or oversee the preparation of reports regarding specialty care. E. Reviews clinical outcomes, progression of care, and financial indicators with appropriate Medical Directors and other specialty physicians. F. May provide education for members of the interprofessional team regarding special needs in the care of specialty patient population. G. Acts as a consultant to other CareLines, service lines, BPTs on specialty specific patient care needs. Quality Improvement and Research A. Recommends, supports, participates or leads quality improvement initiatives regarding specialty care. B. Description The ICM Nurse Lead serves as a senior nurse case manager clinical leader within the Intensive Case Management program and is responsible for providing day-to-day clinical guidance, case consultation, care coordination oversight, workflow support, and clinical escalation management for complex and high-risk patients. The ICM Nurse Lead supports the delivery of a high-touch, longitudinal, interdisciplinary case management model designed to improve patient outcomes, reduce avoidable emergency department visits and hospitalizations, facilitate safe transitions of care, address social determinants of health, and improve coordination across the healthcare continuum. The Nurse Lead serves as a critical bridge between ICM leadership, Nurse Case Managers, Social Workers, Patient Navigators/Care Coordinators, physicians, specialists, inpatient teams, SNFs, home health, behavioral health, and other community partners. The position promotes consistent application of ICM standards, evidence-based clinical practices, acuity stratification, outreach expectations, documentation standards, interdisciplinary collaboration, and transition-of-care processes. The position plays a critical role in facilitating timely access to care, reducing barriers across the continuum, and supporting effective care transitions through close collaboration with interdisciplinary teams, including Nursing, Social Work, and Utilization Management. This role drives operational excellence through process improvement, data-informed decision-making, and staff development, contributing to improved patient engagement, quality outcomes, and overall program performance.
