Assists the patient in room maintenance, such as bed-stripping and trash removed, when necessary • Performs on-to-one observation or close observation status of patients, e.g. safety, elopement risk, or suicide prevention. • Maintains “isolation status” for patients under direction of an RN and according to set procedures • Performs scheduled hourly care checks/ rounding on patients with emphasis on caring for toileting needs, comfort, and positioning and pain awareness. Promote and encourage healthcare activities such as health screening, health assessments, health care linkages, and health presentations. Makes telephone calls and home visits to individual residents aimed at assessing their needs and supporting successful community living activities. Makes referrals to appropriate health services based on the assessed needs of the resident. Provides Health Care Advocacy by acting as a liaison between the various healthcare systems during times of transition. Assist residents needing direct patient care to connect with their physician or other appropriate health care services to address their individual needs. Documents all nursing interventions provided as required by site location. Participates in special projects and performs other duties as assigned.
