Job Summary and Responsibilities Thiis is a remote position requring travel to support enterprise wide Virtual Health Services As our Virtual Integrated Care Professional, you will deliver comprehensive, coordinated health services remotely, leveraging technology to connect with patients and facilitate seamless care across various disciplines. Every day you will engage patients through telehealth and other digital tools to conduct assessments, provide education, manage chronic conditions, and coordinate care plans with multidisciplinary teams. To be successful in this role, you must possess strong communication and clinical assessment skills, comfort and proficiency with virtual healthcare technologies, an ability to work autonomously and collaboratively, and a commitment to patient-centered, integrated care.
