Nurse Practitioner, Longevity Care in Senior Living (In-Person Contract)

About Total Life Total Life connects adults 65+ with licensed clinicians for behavioral health, geriatric care and longevity services, covered by Medicare, Railroad Medicare and major commercial insurers. Since 2021 we've grown to more than 200 credentialed providers serving patients in 49 states. Most longevity medicine is built for healthy 40-year-olds. Ours is built for older adults. The Longevity Program combines a comprehensive geriatric assessment with chronic disease management, cognitive health, medication review and weight management, organized around five pillars of healthy aging: sleep, nutrition, movement, mental health and social connection. It works alongside a patient's existing care and never replaces their primary care provider. The role This is an in-person contract role. We're looking for nurse practitioners in the Chicago suburbs (Joliet-Palatine) area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits. You'll be trained and supported by our Chief Medical Officer, a board-certified geriatrician who designed the program's clinical content. Your first site day is run alongside a member of our team. Schedule Site days are on weekdays. The presentation is the fixed part of the day. The rest depends on how many residents sign up. Expect roughly 4 to 8 hours per site day, a few times a month to start, with hours growing as bookings grow. Assists the Wound Care Nurse as directed. 9. Completes medical records documenting care provided and other information in accordance with nursing policies while maintaining strict confidentiality. 10. Must answer and respond to call lights promptly and courteously. 11. Assist with new admissions, re-admissions and assist with the transfer of Guests to different rooms within the facility. 12. Overview Department: Recovery Room Schedule: Full-Time, 11:00am - 7:30pm / on-call requirement Hospital: Resurrection Medical Center Location: Chicago, IL 60631 Responsibilities The PACU RN is accountable directly to the OR Charge Nurse and the Director of Surgical Services, and indirectly to the House Supervisor. The Registered Nurse is responsible for the delivery of patient care through the nursing process of assessment, planning, implementation, and evaluation for all patients within the PACU scope of services. The Registered Nurse also coordinates the post-operative care of the patient with the surgeon and anesthesiologist, directs and guides other personnel while maintaining standards of care in the PACU. He/She communicates with the physician about changes in the patient’s clinical condition including hemodynamic monitoring, results of diagnostic studies and abnormal results and symptomatology. Works with other departments and co-workers to provide quality patient care across the continuum, efficient patient flow and unit maintenance and organization. Responsible for adherence to regulations, standards and hospital or unit policies and procedures. Responsible for participation in Performance Improvement activities, unit meetings and in-services. Acts as a resource for other personnel as appropriate. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. Current and valid state RN License. 2. Current BLS (AHA) certificate upon hire and maintain current. 3. ACLS (AHA) certificate 30 days upon hire and maintain current. 4.
