Ensure chart is available, with relevant lab work and test reports if indicated, for physician when the physician sees the patient. • Maintains an awareness of current patient centered evidence-based practices and uses knowledge of applicable research data in provision of patient care. • Collaborates with members of the interdisciplinary team and promotes collegial relationships. • Assists patients and communicates with licensed providers to obtain prescription renewals as needed. • Performs patient related functions as directed by the Licensed Provider and/or Registered Nurse (RN), and documents properly. These may include the following, as governed by LPN scope of practice: medication administration, vaccination, other injections, and patch placement, set up and/or assist for procedure. • Communicates directly with patients regarding their care or treatment using appropriate terminology and respectful manner. Interacts with medical staff efficiently. Minimum Qualifications: To qualify you must have a Graduate of LPN program with current NYS OP LPN licensure and registration. Ability to complete multiple tasks efficiently and thrive in a team work environment which pursues a positive patient care experience. Physical stamina, manual dexterity, visual and aural acuity to perform responsibilities needed also required..
