Job Description Job Function Summary: Involves strategic planning, organization, and general management for population health, delivery system reform / innovation and accountable care. Assures proactive response to local, state and national initiatives and supports the development of a highly reliable care delivery system. Works with medical staff, department chairs, and medical center leadership in developing and implementing delivery system reforms. Custom Scope The Assistant Clinical Director (ACD) is a highly visible and influential leadership role within the Office of Population Health (OPH) at UCSF Health. As a key member of a dynamic and mission-driven leadership team, this individual brings a strong leadership presence, sound judgment, and a commitment to operational excellence in advancing innovative models of care. This role provides comprehensive operational oversight for the following disciplines: Nursing Supervisors/Managers, Registered Nurses, Advance Nurse Specialists, other Nursing positions, along with other disciplines across a diverse and growing portfolio of programs, with a focus on Complex Care Management, Transitions, and other Care Coordination Programs for both Adults and Pediatrics.In addition, the ACD shares accountability for OPH Community Events and provides joint leadership for the OPH Pharmacy Program (e.g., Vaccines for Children and community-based vaccine initiatives). The portfolio may continue to expand in alignment with OPH and UCSF Health strategic priorities. The ACD delivers strategic, operational, and administrative leadership across multiple population health initiatives. This leader is responsible for translating vision into execution—developing actionable plans, optimizing performance, and achieving measurable outcomes that advance equitable, high-quality care. Core areas of accountability include financial stewardship, workforce leadership, regulatory compliance, and the design and implementation of standardized, scalable processes. Job Description The University of California, San Francisco Heart & Vascular Center’s Advanced Heart Failure Comprehensive Care Center is seeking a full-time Advanced Practice Provider (APP) for a night-shift position to care for patients with advanced heart failure, mechanical circulatory support (MCS), heart transplantation, and other complex cardiovascular conditions. The APP will join a highly experienced multidisciplinary team of physicians, surgeons, APPs, nurses, pharmacists, social workers, and researchers who provide comprehensive care for advanced heart failure patients across the continuum of care. UCSF’s Advanced Heart Failure and MCS programs are nationally recognized for delivering cutting-edge therapies and support in both inpatient and outpatient settings, including a wide range of temporary and durable mechanical circulatory support options. Serving as a primary provider in the hospital setting, the APP will collaborate closely with Advanced Heart Failure Cardiologists, Cardiothoracic Surgery and Intensive Care teams, as well as nursing, case management, social work, and other multidisciplinary partners to evaluate, treat, and manage this highly complex patient population in both Intensive Care and Acute Care Units. This position is primarily night-based and offers the opportunity to practice at the forefront of advanced heart failure, transplant, and MCS care within a leading academic medical center. Responsibilities 60% Inpatient - Direct Patient Care (may include some or all of the responsibilities listed) 1.1.Independently (independent APP encounter) or collaboratively (shared physician and APP encounter) evaluates and manages patients. Includes taking histories, performing physical examinations, responding to changes in patient status, performing consults, ordering or furnishing medications/diagnostic tests/treatments/therapies/consultations/referrals/durable medical equipment (DME)/home health services. Care provided must be documented in the electronic medical record in accordance with the UCSF Rules and Regulations. Provider services must be billed to reflect the performing provider and comply with the APP Billing Policy 3.08.01. APPs must respond to clinical documentation integrity inquiries from the Department of Quality and Patient Safety within 3 days. 1.2. LVN Supervisor - Berkeley Outpatient Multispecialty Center

Job Description Under the general supervision of the Nurse Manager, the Licensed Vocational Nurse (LVN) Supervisor will oversee clinic operations and will have primary responsibility for the supervision of LVNs and medical assistants at UCSF’s Berkeley Outpatient Multispecialty Center (BOPC). They will also have shared, indirect oversight over other support staff such as practice coordinators in coordination with the Practice Supervisor/Administrative Director. The Licensed Vocational Nurse (LVN) Supervisor will have clinical and administrative responsibility for developing and maintaining a high standard of patient care. Job Description UCSF nurse practitioners (NPs) are advanced practice providers (APPs) who possess the education, national board certification, California licensure, knowledge, clinical skills, and experience necessary to provide evidence-based patient care in their area of clinical practice. They provide routine, acute, critical, and specialty care as defined by their granted privileges/standardized procedures. These may include performing non-invasive or invasive procedures and operative first assist. Organization: CPMC-California Pacific Med Center - Mission Bernal Position Overview: Manages and provides operational and nursing leadership ensuring the quality of nursing care within preoperative, intraoperative, and postoperative areas. Plans and organizes operations and resources to ensure effective and efficient delivery of patient care consistent with applicable laws, standards and regulations. Provides patient care and acts as a resource person to the staff and assures patient care needs are met in the department. Demonstrates critical thinking skills and, in collaboration with other disciplines, surgeons, anesthesiologists, is able to coordinate unit activities, staffing plans and contributes to resolving inter and intra departmental problems. Assesses the quality of care delivered and provides input for staff mentoring/evaluation, staffing, hiring, performance management and general daily operation of the unit.Establishes protocols and practices, ensuring compliance with department, affiliate, operating unit, and/or system standards, policies and procedures, the Joint Commission (TJC) standards, and applicable laws and regulations. Demonstrates understanding of business and operational requirements. Demonstrated leadership skills, including team building, and coaching/mentoring with the ability to motivate and engage others. Demonstrates solid clinical proficiency and knowledge to effectively plan and coordinate patient care consistent with applicable laws, standards and regulations. Working knowledge and thorough understanding of state/federal/local and/or accreditation standards and requirements as it relations to operations and organization, with the ability interpret and implement applicable standards and requirements. Demonstrates understanding of healthcare financial environment. Demonstrates ability to develop and manage a budget. Understands age specific needs in providing care to patient population served. Demonstrates tactical and analytical thinking and planning skills, and is able to develop plan to accomplish objectives, identify obstacles and resolve operational issues.
