Job Summary Provides direct and indirect patient care in the office setting to patients, families and caregivers in the Medical Group practice. Uses the nursing process consistent with acceptable standards of the profession and MVHS Medical Group. Provides high quality patient centered care that reflects initiative, flexibility and responsibility with a minimum amount of administrative supervision. Determines priorities of care based on physical and psychosocial needs, as well as factors influencing patient flow through the system. Communicates with outpatient clinic physicians and NP/PAs (healthcare providers) about changes in patient’s status, symptomatology and results of diagnostic studies. Overview To support the Mission of Helio Health, Inc., the CTI Licensed Practical Nurse will provide comprehensive care management and coordination of services to Critical Time Intervention (CTI) program enrollees. The CTI model is a time-limited form of case management that focuses on helping people with severe mental illness (SMI) and a history of homelessness during a “critical time” transitional period in their lives. CTI Licensed Practical Nurse: CTI is designed specifically to enhance the continuity of care during transition from hospital to community care.
